- Pre-Solicitation Notice Amendment to Solicitation VA-260-09-RQ-0280 for Veteran's Affairs Home Lab/Blood Draws. This FedbizOpps Notice is being amended to facilitate the transmission of some answers to vendor questions regarding the requirement, (see the actual solicitation amendment for details), and to make a change to the Statement of Work (SOW). Offerors must consider the answers and the changes referenced in the solicitation amendment. Offerors must also do one of the following: 1. place your company address information in block 8, the typed or printed name of the signer in block 15A, the signature of the signer in block 15B, and the date the signer signs the amendment in block 15C, scan the signed amendment into your computer and send it by email, or fax the signed amendment to Dennis Riches at 360-256-0936, or 2. reference acknowledgement of this amendment in your proposal in the "Acknowledgement of Amendments" Section of the initial solicitation document, of which you have signed and dated.
