Description
The 97th Contracting Squadron at Altus Air Force Base, Oklahoma, hereby issues this Request For Quote for an Intercom System Replacement for Building 278, 97th Maintenance Directorate, Altus AFB. Quotes are being requested for solicitation number F1B0H19071A001, and a written solicitation will not be issued. Contractor shall provide a quote by 12:00pm CST, Friday, 10 April 2009 that includes price, product desription, delivery estimate, and past performance. EVALUATION FACTORS This is a competitive Total Small Business Set-Aside IAW FAR 19.502-2, Total small business set-asides. Each offeror shall complete paragraph (j) of the provisions at FAR clause 52.212-3, Offeror Representations and Certifications—Commercial Items (Mar 2005) if applicable. Please register Representations and Certifications on ORCA, http://orca.bpn.gov/. simplified procedures authorized by FAR 13.5 “Test program for Certain Commercial Acquisitions”. For quotations determined technically acceptable, a best value selection based on price and past performance will be made. An offeror will be determined technically acceptable if no exception is taken to the specifications and requirements of the RFQ. Award will be based on both price and past performance. Each offeror’s quoted price will be evaluated for reasonableness. Price will always be an important factor; therefore, offerors should make diligent efforts to control costs and submit fairly priced quotes. Past performance will be evaluated based on relevancy (size/scope/complexity), quality of product/service, timeliness of delivery/performance and business relations (customer service). Relevant past performance must have occurred within the past 5 years. To be considered for award, an offeror must have a satisfactory or better past performance record based upon information available to the contracting officer (Contractor Performance Assessment Report) and offer a reasonable price. The offeror may be asked to submit at least two references that can provide relevant past performance information if the contracting officer cannot locate CPAR information. Award may be made to other than the low offeror when, after evaluating past performance, it is determined by the Contracting Officer to be in the Government’s best interest. STATEMENT OF WORK Full-time Roman Catholic Priest 10 March 2009 15 April – 7 September 2009 Altus AFB, OK NON-PERSONAL SERVICES: The contractor shall provide all transportation and labor to perform duties of a purely religious nature, such as conducting religious services, pastoral counseling, and religious ministrations and rites as a Roman Catholic Priest for Altus Air Force Base Chapel. Period of Performance is 15 April – 7 September 2009 1.0QUALIFICATIONS a.Knowledge of Scripture, teachings of the Roman Catholic faith, and Canon Law. b.Knowledge of the role of the Catholic priest serving in an adverse and pluralistic military environment. c.Thorough understanding of, and extensive pastoral experience in, the special problems involved in a military community. d.Knowledge of, or ability to learn, the structure, functions, practices, and rules of a military base. e.The priest is required to be in good standing with his Diocese or religious community, have the permission of his Ordinary/Superior and have endorsement of the Archdiocese for the Military Service. f.Be able to use Microsoft, the Internet, and email. In addition, the individual must also be able to function correctly within the CTOF (Chapel Tithes and Offering Fund) funds process and the AFI/OI constraints. g.The priest is required to have strong oral and written communication skills and be able to work with various chapel committees and groups, both Protestant and other faiths. h.Furnish a resume including academic and work-related references. Scope of Responsibilities. Provides management of the Catholic program, catholic religious services, and Catholic rites and rituals. Maintains a liaison with the Roman Catholic Church at diocesan, archdiocesan, and national levels to meet the needs of the Altus Air Force Base Catholic military personnel, their families, retirees, and civilian contractor’s work. This is a full-time position. The priest must be physically in the office at least 40 hours a week. 35 hours Monday- Friday, and 5 hours Saturday and Sunday, to include a minimum of 3 daily masses and Holy days. Will conduct funerals for registered parishioners and active duty military. The specific duty schedule will be coordinated between the Wing Chaplain, or his/her designee, and the contractor. Provide Catholic worship, liturgy, rites, and pastoral care. Establish periods of Catholic worship, provide the proper liturgical environment, and conduct scheduled Eucharist services as well as devotions, prayer services, sacramental rites and funerals. Initiate development of special services in times of national or military crisis and mourning or national celebration and thanksgiving. Prepare homilies/sermons of the highest quality. Provide sacramental ministry to all Roman Catholic military members and their family members to include a full Religious Education program. Provide leadership for scheduled chapel worship services. Officiate and minister necessary hospital services for military members and their family members. Create an environment that encourages Catholic fellowship and promotes the common values that support a military parish. 2.0General Responsibilities: a.Exercise management oversight responsibilities for the Catholic parish in the Chapel. Advise and provide counsel to contractors and volunteer staff regarding policies, procedures, programming, and directives of management. Explains expectations to contractors and volunteers. Identifies contractor and volunteer developmental needs and provides or arranges for training (formal and on-the-job) to maintain and improve job performance. Provides priest oversight of a religious educational program in accordance with the guideline set forth by the Air Force Chaplain Service and AMS. Submit accurate and timely reports and perform required administrative tasks. If awarded contract, the contractor will upon request complete (IRC), AF Form 2583, items 1-7 only, the Volunteer Questionnaire (Atch 8), the Acknowledgement of Rights and Consent To Release Records Letter (Atch 2), and provide a signed statement acknowledgement of Chapel Child Protection Policy (Atch 1). Additionally, an Oklahoma state background check will be accomplished, in addition to a federal background check by AFOSI. These background checks will be accomplished at the expense of the chapel. The contractor must also initiate a National Agency Check if required in accordance with HC OI 52-9. Failure to accomplish a favorable background check IAW DODI 1402.5 and HC OI 52-9 may result in termination of contract in accordance with FAR 52.212-4. 3.0Specific Responsibilities: a.In consultation with the Wing Chaplain, develop with the Catholic Parish Advisory Council, an annual calendar of events, CTOF and appropriated fund budgets, and submit these items to the Wing Chaplain as determined by local instructions. b.Perform Catholic sacraments or subcontract a priest or deacon with faculties to provide Catholic sacraments as requested by Active Duty members and their families. The priest is required to coordinate any changes from the above requirements with the Wing Chaplain or their designee. c.Keep a record of attendance and rites performed and submit these records on the monthly chaplain Service Air Force Form 1270 reports and the quarterly AMS report according to AFIs and AMS instructions. d.Hold monthly planning meetings with the Catholic Parish Advisory Council. eAccountable for paying any Social Security payments, federal or local income taxes, and state unemployment insurance premiums for themselves, as well as any individual employed or subcontracted by them to provide services under this contract. f.The contractor will personally perform the services described above or coordinate performance by a substitute of equal or higher proficiency who has been approved in advance by the Wing Chaplain or their designee. The contractor will submit the names and qualifications of proposed subcontractors to the Wing Chaplain for approval at least 14 days prior to the proposed absence. Sub-contractors must meet the requirements for the position as outlined in this Statement of Work (SOW). Contractor is responsible for payment of substitute. The contractor will only be paid for services rendered and received by the government. g.The priest is required to coordinate any changes from the above requirements with the Wing Chaplain or their designee. h.The priest is required to report 15 minutes prior to any scheduled worship service or activity and finish the mass or activity within the proscribed time dictated by the Chapel schedule. i.The priest may be required to provide emergency sacramental coverage or preside over Roman Catholic rites per coordination with the Wing Chaplain. j. The Wing Chaplain (or his/her designee): Provides Chapel Tithes and Offering Fund (CTOF) training at least once per year. k.Will monitor and inspect the contractor’s performance. The contractor will be notified in writing of all noncompliance work, and given an opportunity to correct nonconforming work. The contractor will be given an opportunity to response in writing to any notice of noncompliance. END SOW 4.0 DRESS AND APPEARANCE a.The priest is required to dress in appropriate attire for worship services or other special religious observance, as required by the Archdiocese for Military Services. 5.0SERVICES/PAYMENTS a. To receive payment for services, contractor will need to submit an invoice electronically through Wide Area Workflow. Payment can take up to 30 days. 6.0GOVERNMENT FURNISHED SUPPLIES/EQUIPMENT a.The contractor shall have access to the chapel administration equipment (i.e. copier, computer, fax machines, etc.), be provided with office space with desk, chair, computer, printer, filing cabinets, bookcases, phone, expendable office supplies and other items deemed necessary to conduct the coordination of this contract, as needed and available. b.The contractor shall have access to reprographics and mailing support as approved by the Wing Chaplain or Chapel staff. Award shall be made only to contractors who have registered with the Central Contractor Registration at: www.ccr.gov/ <http://www.ccr.gov/>.
