Description
Section B-- Tables B #1-12 Attachments J-1 thru J-23 RFP dated April 2, 2009 SUBJECT: RFP for Operation and Maintenance Services Contract for U.S. Embassy Europe/Asia, SAQMMA-09-R-0037 At this time we are requesting proposals in response to the RFP issued on April 2, 2008. All proposals shall be submitted IN THREE VOLUMES and the correct number of copies (please see the matrix below) on June 4, 2009 no later than 2:00 pm and in accordance with the Section L and M - Instructions and evaluation method factors included in this RFP. Five (5) sets (Original and 4 copies) shall be submitted: VOLUME 1 Volume I / Tab A: Cover Letter and Executive Summary Volume I / Tab B Factor 1: Key Personnel VOLUME II Volume II / Tab C Factor 2: Post Operation Plan and Quality Control Volume II / Tab D Factor 3: Past Performance Volume II/ Tab E Factor 4: Ability to Recruit, Train, and Retain High-Quality Personnel Volume III Volume III/ Tab F: Contract Documents and Associated Information Volume III / Tab G Factor 5: Price Proposal Volume III / Tab H Factor 6: Financial Statements Please refer to Section L.14 Proposal Preparation Instructions of this RFP for the detailed proposal preparation instructions. Your proposal must be prepared and submitted in accordance with the directions provided. The offeror shall state, explain, and justify any deviations, exceptions, or conditional assumptions taken regarding any of the instructions or requirements of this solicitation in the appropriate volume of the offer. Proposals shall be submitted to Alla Weinstein at the address indicated at Block 8 of Standard Form 33: U.S. Department of State, Office of Acquisition Management (AQM), P.O. Box 9115, Rosslyn Station, L-600 Arlington, VA 22219 The site visit and pre-award conference will take place in Moscow, April 20 and 21, 2009, Beijing April 23 and 24, 2009 and Hanoi, April 27, 2009. A maximum of two representatives per company may attend the Site Visit. All offerors must make their own travel arrangements (hotel, flights, etc) to be on site for the site visit. No government assistance is to be expected with travel. However the program office will send the country clearance notice to get offerors access to the site. The cost of this visit is to be considered the cost of doing business, no stipend will be paid. Please note, that Final Secret clearances are required for the walkthroughs. Interim Secret clearance will not be acceptable. Country clearance will be required for ALL contractor travelers prior to the trip. Country clearance information (CCR) must be submitted to Alla Weinstein, Contract Specialist, no later than 12PM noon on April 13, 2009. The notice shall specify the following information so that a country clearance notice can be sent authorizing access to the site. Name Dates of travel (departure/return) DOB POB SSN Passport # Passport date of issue Passport date of expiration Clearance Level You are cautioned that the Contracting Officer or Contract Specialist is the only individual authorized to provide guidance and respond to questions concerning the solicitation or its requirements. Contractors are encouraged to ask questions and seek clarifications concerning the solicitation. Do not rely on information from any other source. If you have any questions about the RFP, its requirements, or any aspect of the work that may be required, submit these directly to Alla Weinstein at weinsteinal@state.gov or at 703-875-4943. All questions must be submitted IN WRITING no later than 12 PM noon on May 14, 2009.
