Send completed forms to the following address: EASTERN AREA OFFICE SOLICITATION NO. 150-68-09 5315 CAMPBELLS RUN RD PITTSBURGH PA 15277-7070 Bidder must make sure to include the Solicitation number in the address. All documentation for proposal must be received by the above address no later than April 30, 2009 by 3:00 pm. Provide a photocopy of your Social Security Card or IRS Form CP 575 (EIN) *REQUIRED* ***At time of award vehicles must be 2007 model year or newer*** NO SOLICITATION PACKAGES WILL BE SENT FROM THE CONTRACTING FACILITY OR THE AREA OFFICE.
