Description
The Oklahoma City VA Medical Center, 921 N.E. 13th Street, Oklahoma City, OK, is issuing a solicitation for Specialized Medical Resource Sharing Agreement for Primary Care Services Community Based Outpatient Clinic (CBOC). Services shall be for an estimated 650 veteran patients residing within a 30-mile radus of Stillwater, Oklahoma. The following services are included in the solicitation: Primary, post hospital care, radiology, laboratory services and urgent first fill prescriptions to treat acute conditions. Services of the CBOC shall be made available five days a week Monday through Friday during normal clinic operating hours. The North American Industrial Classification System (NAICS) is 621110 and the size standard is 5M. It is the VA's intention to negotiate and award a single contract for Primary Care Services. This contract will be awarded on a competitive basis under the authority of Title 38 U.S.C. Section 8153 pending availability of funds. The contract(s) will be for a period of one year from the date of award with four one-year renewal options that can be exercised at the option of the Government. Costs for primary care services should represent an "all inclusive" reimbursement for routine delivery of primary health care. Offerors should quote a capitation rate per enrolled patient for a period of one month. The yearly costs should include the cost per patient per month x 12 months x estimated number of patients.. All requests for bid packages and/or responses to this notice must be submitted via email to Beverly.Kimball@va.gov. Telephone requests/responses are not permitted. Bids must be submitted for the entire contract period and must be complete in order to be considered. Bids shall be due back in this office no later than 4:30pm Friday, May 1, 2009.
