Description

The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Purchase Order for a Nephrologist to provide services at the Pine Ridge PHS, IHS Hospital in Pine Ridge, South Dakota. Contractor will work two (2) clinic days per month from 8:00am- 4:30pm; days to be determined by contractor. The period of performance will be from May 11, 2009 through December 31, 2009. The Contractor shall perform according to the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-09-073, shall be in writing either by email or fax. In the written request please reference RFQ number RFQ-09-073 and provide a postal mailing address, email address and state preference as to how the RFQ package can be sent. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be April 17, 2009. The closing date of the RFQ package is May 4, 2009. This is small business set-aside.
