The following information will be needed to complete forms that will be returned with your proposal; HCR # 45684 Solicitation # 150-70-09 Date of Solicitation – 04/03/2009 Begin Contract Date - 07/01/2009 End Contract Date – 06/31/2013 For Mail Service Between City, St – Rio Grande, OH City, St – Patriot, OH Annual Hours – 252,156 Annual Miles – 5,212.8 Attachment labeled " Forms Contractor must Complete and Return" will need the above information and be fully completed by the contractor. Completed forms will need to be sent to the following address; EASTERN AREA OFFICE SOLICITATION NO. 150-72-09 5315 CAMPBELLS RUN RD PITTSBURGH PA 15277-7070 Contractor must make sure to include the Solicitation number in the address. All documentation for proposal must be received by the above address no later than May 05, 2009 by 15:00 pm. Provide a photocopy of your Social Security Card or IRS Form CP 575 (EIN) *REQUIRED* NO SOLICITATION PACKAGES WILL BE SENT FROM THE CONTRACTING FACILITY OR THE AREA OFFICE.
