- This is a Highway Contract Route Solicitation. The following information is required to complete forms included with your proposal: HCR # 159M2 Solicitation # 150-68-09 Date of Solicitation – 4/1/2009 Begin Contract Date – 7/1/2009 End Contract Date – 6/30/2011 For Mail Service Between City, State – Johnstown P&DF, PA City, State – Dunlo, PA The attachment labeled “Solicitation Package 1" requires the above information; all forms included in this package must be fully completed by the bidder. Send completed forms to the following address: EASTERN AREA OFFICE SOLICITATION NO. 150-68-09 5315 CAMPBELLS RUN RD PITTSBURGH PA 15277-7070 Bidder must make sure to include the Solicitation number in the address. All documentation for proposal must be received by the above address no later than April 30, 2009 by 3:00 pm. Provide a photocopy of your Social Security Card or IRS Form CP 575 (EIN) *REQUIRED* NO SOLICITATION PACKAGES WILL BE SENT FROM THE CONTRACTING FACILITY OR THE AREA OFFICE. ALL DOCUMENTATION MUST BE DOWN LOADED AND PRINTED FROM THE ATTACHMENT TO THIS SOLICITATION. NO PAPER COPIES WILL BE MAILED.
