Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.he request for proposal is being issued as unrestricted. Small businesses are encouraged to respond under NAICS 621498 in accordance through FAC 2005-15. The contract shall be awarded as an indefinite-quantity, indefinite-delivery type contract with a Base Year with 4 Option Years. The minimum order amount will be $25,000 with a maximum order amount of $1,000,000.The contractor shall provide qualified Louisiana licensed personnel, material to perform the services, and /or OSHA, HIPPA, ADA, and Privacy Act Compliant Mobile, Self-Contained Health Practices depending on the individual site needs for the LAARNG. "Medical evaluation by a Louisiana licensed provider (Physician, Physicians Assistant (PA), or Nurse Practitioner (ARNP) IAW prescribed Army and NGB policy for Periodic Health Assessment, Physical Exams (Over-40, flight, etc.). "Exams will be entered into the PHA module of MODS or an applicable substitute (ex&NGB Overprint) to be provided by the Office of the State Army Surgeon, LAARNG. "Results entry and input/entry of all medical related FMR/IMR functions into the MEDPROS module of MODS. "Audiologist / SPRINT Testing for all applicable Soldiers. "Perform Pap-Smear examinations and MEDPROS entry for all LAARNG Soldiers. "Annotation and entry of services into the DD Form 2766 Adult Preventive and Chronic Care Flowsheet or appropriate document. "Clinical evaluation by a licensed dentist of the oral cavity using a visual, clinical exam supported by bitewings taken by dental technicians. 3. REFERENCES (http://www.usapa.army.mil/) "AR 40-501 Standards of Medical Fitness "NGB Memorandum Post Deployment Health Re-Assessment Screen (attached) "NGB Memorandum Periodic Health Assessment Interim Guidance (attached) 4. SERVICES The contractor shall provide all management, labor, material, equipment, certifications and supplies necessary to provide all listed services in a mobile, HIPAA, Privacy Act, OSHA & ADA compliant, stand-alone environment. An undetermined number of each services listed in the SOW will be delivered throughout the period of performance, at multiple armories with the possibility of simultaneous requirements, to be determined in each delivery order. The services shall provide mandated medical requirements to meet readiness and deployability standards to ensure mission capability of the LAARNG. 5. GOVERNMENT-FURNISHED PROPERTY AND INFORMATION: Only parking will be provided for HIPAA, Privacy Act, OSHA& ADA-Compliant, Self-Contained Mobile Medical Practices. No armory, TMC, or office space will be provided or utilized unless prior approval from the Office of the State Army Surgeon. 6. CONTRACTOR FURNISHED ITEM: Contractors shall provide all items and licensed personnel needed to perform the Medical & Dental Readiness requirements, any employee benefits, vacation days, paid Holidays and sick leave plan. Mobile Medical Practice support shall meet all OSHA, ADA, & HIPAA standards to ensure a safe and productive workspace for medical services under the Individual Readiness Requirements. The Mobile Medical Practice must contain minimally 3 fully equipped & private medical exam rooms, designated vitals area, visual acuity area, a lab with eye-wash stations, biohazard separated work areas, hearing booth with ISO-Booth for SPRINT testing, and handicap accessibility. Mobile Medical Practices shall be a fully functional medical office whereby no additional space will be given inside military buildings during events as these events may be in remote areas such as artillery ranges. The contractors safety program shall include equipment training, occupational safety, health, and fire protection guidance to ensure the protection and health of all work personnel. Due to the size of the LAARNG force and need for very large, short-termed events, Contractor shall possess an owned & operated fleet of mobile health practices containing a minimum of 20 vehicles to accommodate multiple large events at the same time. Contractor must have all capabilities to include Medical & Hearing vehicles. All vehicles shall be OSHA/ADA/HIPAA compliant and have wheel chair access hallways and an onboard wheel chair lift. Internet connectivity must be available for any Mobile Health Practice deployed to allow for real-time data entry at events. Patient Education screens are ideal to ensure soldiers receive continuing education. Due to nature of many drill weekends, annual training events, and a busy schedule for soldiers, Mobile Health Practices must be able to be moved at short notice of a minimum of 1-hour notice to follow movement of patient base to be served. Government property shall also be protected during the period of time the property is under the control or possession of the contractor. 7. SPECIFIC TASKS: 7.3 Periodic Health Assessments (PHA): The contractor shall conduct PHAs in accordance with the PHA interim guidance set forth by the National Guard Bureau of the Surgeon General (See Appendix D) for an undetermined number of soldiers at undetermined armories and training sites throughout the State of Louisiana. The contract must utilize either OSHA-Compliant Mobile Health Practices and/or sites determined by the LAARNG. Compliance with OSHA, State & Federal Regulations is mandatory. 7.4 Post Deployment Health Re-Assessments (PDHRA): the contractor shall conduct PDHRAs in accordance with NGB memorandum (See Appendix D) set forth by the Office of the Surgeon General for an undetermined number of soldiers at undetermined armories and training sites throughout Louisiana. The contractor must utilize either an OSHA & ADA Compliant Mobile Health Practice and/or sites determined by the LAARNG. Compliance with OSHA, State & Federal Regulations is mandatory. 7.5 Periodic Health Assessments and Physical Exams: the contractor shall conduct Periodic Health Assessments (PHA) and other physical exams (Over-40, Chapter 2, Flight, etc.) in accordance with the AR 40-501 for an undetermined number of soldiers and undetermined armories throughout Louisiana, which will be identified in each delivery order. The contractor shall conduct physicals in an HIPAA, Privacy Act OSHA & ADA Compliant Mobile Health Vehicle to include visual acuity, ANCI certified and DOEHRS compatible eudiometry, on-site laboratory testing for lipids, glucose, urinalysis, hcg, and hemoglobins, a complete physical exam by certified clinician, tonometry, and vital signs. All professionals shall be licensed and adhere to all requirements of OSHA, State & Federal Regulations is mandatory. 7.6 Visual Acuity Testing: The contractor shall conduct visual acuity testing in accordance with AR 40-501 for an undetermined number of soldiers and undetermined armories throughout Louisiana, to be determined in each delivery order. Adherence to OSHA, State & Federal Regulations is mandatory. 7.7 DOEHRS Compatible Audiometer Testing: The contractor shall conduct audiometry in accordance with AR 40-501 for an undetermined number of soldiers and undetermined locations throughout Louisiana. Audiometry must be DOEHRS compatible. Adherence to all requirements of OSHA, State & Federal Regulations is mandatory. Audiology Evaluations and Speech Recognition in Noise Test (SPRINT) testing will be performed IAW all applicable Federal, DoD, MEDCOM, NGB, and State guidelines. 7.8 Immunizations: The contractor shall conduct immunizations in accordance with AR40-501 for an undetermined number of soldiers at undetermined locations throughout the State of Louisiana, to be determined in each delivery order. Immunizations shall include, but not limited to, flu, yellow fever, typhoid, tetanus, hepatitis A, hepatitis B, and polio. Adherence to all requirements of the OSHA and all State & Federal Regulations is mandatory. 7.9 Phlebotomy/Lab: the contract shall conduct all phlebotomy services such as HIV and G6PD blood draws, pregnancy testing, pap-smear testing, and lipid profiles in accordance with AR 40-501 for an undetermined number of soldiers in an undetermined number of armories & training sites throughout Louisiana, to be determined in each delivery order. All specimens will be turned over to the Laboratory that is currently under contract with the LAARNG or NGB for testing. The contractor shall utilize the current accepted shipping method. 7.10 DNA Collection: The contractor shall conduct DNA collection in accordance with the AR 40-501 for an undetermined number of soldiers in an undetermined number of armories throughout the state of Louisiana, to be determined in each delivery order. The contract will utilize the LAARNG Medical Detachment for all DNA supplies, shipping supplies, and shipping guidance. Adherence to all requirements of OSHA, State & Federal Regulations is mandatory. 7.11 Event & Post-Event Case Management of Audiology specific conditions/services: "Contractor shall setup network & account with audiologists near event location "Contractor Audiologist to input all Category 2 and Category 3 required Hearing Profiles (H2 or H3) into the MND Module of MODS IAW established template provided by the Office of the State Army Surgeon and AR 40-501 "Contract Case Manager will identify, track, and provide clinical oversight of Category 2 and Category 3 LAARNG soldiers via hardcopy record/spreadsheet, as well as, the MND module of MODS "When required, Case Manager will coordinate and arrange for fulfillment of required audiology consultations (Audiology Evaluation or SPRINT test) "Government, Soldier, and/or Contractor to provide transportation of soldier to local Audiologists for required consultation service "Contract Case Manager will provide completed Audiology cases to the Office of the State Army Surgeon Clinical Operations Office (or identified representative) for review and approval "Case Manager will provide completed and approved cases, to the Health Services Section of the LAARNG Army G-1 for appropriate processing "Case Manager will maintain oversight on completed required MMRBs and/or MEBs/PEBs for identified Soldiers "Contractor shall be invoiced by & submit payment to subcontracted local Audiologists "Local Audiologists & Contractor to explain hearing issues and applicable instructions to LAARNG treated soldiers and any other POC at the direction of the Office of the State Army Surgeon: "Follow-up conversations between Contractor and LAARNG treated soldier & Supervisor may be necessary depending on complexity of treatment and potential additional requirements "Contractor to collect & tally all expenses of Audiology services & Case management related services "Contractor shall submit costs to authorized Government representative 7.12 Event & Post-Event Case Management of Behavioral Health specific conditions/services: "Contractor shall setup network & account with Behavioral Health Specialists near event location "Contractor will ensure appropriate HIPAA and Privacy Act considerations are in place regarding the sensitive issues of Behavioral Health cases "Contract Case Manager will identify, track, and provide clinical oversight of both Temporary and Permanent Behavioral Health Profiles (P2 and P3) LAARNG soldiers via hardcopy record/spreadsheet, as well as, the MND module of MODS "When required, Case Manager will coordinate and arrange for fulfillment of required Behavioral Health consultations either network or LAARNG organic (Green-Suit) providers "Government, Soldier, and/or Contractor to provide transportation of soldier to local Behavioral Health required consultation services "Contract Case Manager will provide completed Behavioral Health cases to the Office of the State Army Surgeon Clinical Operations Office (or identified representative) for review and approval "Case Manager will provide completed and approved cases, to the Health Services Section of the LAARNG Army G-1 for appropriate processing "Case Manager will maintain oversight on completed required MMRBs and/or MEBs/PEBs for identified Soldiers "Contractor shall be invoiced by & submit payment track, and provide clinical oversight of both Temporary and Permanent Orthopedic Profiles (P2 and P3) LAARNG soldiers via hardcopy record/spreadsheet, as well as, the MND module of MODS "When required, Case Manager will coordinate and arrange for fulfillment of required Orthopedic consultations "Government, Soldier, and/or Contractor to provide transportation of soldier to local Orthopedic required consultation services "Contract Case Manager will provide completed Orthopedic cases to the Office of the State Army Surgeon Clinical Operations Office (or identified representative) for review and approval "Case Manager will provide completed and approved cases, to the Health Services Section of the LAARNG Army G-1 for appropriate processing "Case Manager will maintain oversight on track, and provide clinical oversight of all LAARNG initiated Temporary and Permanent Profiles (T2 and T3, P2 and P3) LAARNG soldiers via hardcopy record/spreadsheet, as well as, the MND module of MODS "When required, Case Manager will coordinate with appropriate specific Case Manager (if applicable) and arrange for fulfillment of required consultations and follow up "Contract Case Manager will report to, and work in detail with the Office of the State Army Surgeon Clinical Operations Office (or identified representative) for disposition and management of Temporary and Permanent Profiles "Case Manager will work with Unit Commanders, senior NCOs, and other unit level leaders to manage Soldier compliance with required consultations independently and/or through the Office of the State Army Surgeon "Case Manager will provide completed and approved cases, to the Health Services Section of the LAARNG Army G-1 for appropriate processing "Case Manager will maintain oversight on completed required MMRBs and/or MEBs/PEBs for identified Soldiers "Contractor shall be invoiced by & submit payment to subcontracted local Audiologists "Local Orthopedic Specialists & Contractor to event location "Upon need of dentally treated soldiers & recommended by licensed dentist, Contractor to arrange for fulfillment of prescription medication for soldier "Government, Soldier, and/or Contractor to provide transportation of soldier to local pharmacy to fill prescription "Contractor shall be invoiced by & submit payment to subcontracted local pharmacy utilized "Local Pharmacy & Contractor to explain prescription medication instructions to LAARNG treated soldier and any other POC at the direction of the Office of the State Army Surgeon "Follow-up conversations between Contractor and LAARNG treated soldier & Supervisor may be necessary depending on complexity of treatment and prescription medication "Contractor to collect & tally all expenses of prescription medication services & case management related "Contractor shall submit costs to authorized Government representative Government will reimburse contractor for invoices of subcontracted prescription medication 8. APPLICABLE DOCUMENTS The contractor shall ensure that all appropriate DA or DD forms (in accordance with references) are utilized for each service supplied. The forms will be made available through the Louisiana Medical Detachment office. The contractor shall provide all documentation concerning medical treatment to the LAARNG. Contractor shall provide proof of Louisiana license to practice to the State Surgeon. 9. PLACE OF PERFORMANCE Throughout entire state of Louisiana. To Be Determined on each delivery order. Contractor must be able to respond within a minimum of 2 days notice anywhere within the state. 10. PERIOD OF PERFORMANCE "To Be Determined on each delivery order. Contractor must be able to respond within a minimum of 2 days notice anywhere within the state 11. OTHER CONDITIONS/ SECURITY 11.1 Safety: The contractor shall establish a safety and health program in accordance with the standards of the Federal Occupational Safety and Health Standard (OSHA), Health Information Portability and Accountability Act (HIPPA), and The Privacy Act. 11.2 Performance work statement is UNCLASSIFIED and under HIPAA guidelines. OSArS shall notify the Contractor if requirement exists for a secret classification and circumstances requiring secret classification for Readiness impact reporting. 11.3 Quality Control: The contractor shall create a quality assurance program that monitors all services being provided. The program must be approved by the LAARNG. 11.4 Hours of Operation: Soldiers will be made available to the contractor during regular drill weekends and training periods between the normal hours of operations to be estimated at 10 hours a day. Usual hours are between 0700 and 1900 unless special arrangements are made. 11.5 Security Requirements: Base security will require all employees and affiliated personnel of the contractor to carry a State recognized identification card or drivers license and all vehicles are subject to search without reason. The contractor shall comply with all installation security requirements. 11.6 Reporting: The contractor shall provide a final report for all services provided to include a by name, unit, and social security number roster of all soldiers receiving services. The report shall be approved and verified by the LAARNG liaison with the office of the State Surgeon of the LAARNG. 11.7. Contractor shall consolidate all medical services performed on an individual event basis and submit an invoice to the Contracting Officer for certification and payment processing. 12. ACCEPTANCE PLAN: All work performed, all materials and tasks data to be submitted under this task will be made available to be inspected for compliance with the SOW and accepted by the COR or authorized representative as specified herein. The following clauses and provisions are applicable to this requirement. 52.212-1 Instructions to Offerors - Commercial Items. 52.217-5 Evaluations of Options. 52.217-8 Option to Extend Services. 52.217-9 Option to Extend the Term of the Contract. 52. 212-2 Evaluation - Commercial Items, factors are as follows: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: The offeror shall provide the following information for source selection. 1. Information concerning the company offering the proposal 2. Technical information concerning mobile equipment to be used to fulfill the contract. 3. Resumes of personnel (health professionals and support staff) key personnel intended to be assigned to work on this effort. 4. Resumes of key administrative personnel intended to work on this contract. 5. Past Performance references: Include the name of the agency/company, telephone number, name of an individual that has knowledge of your past performance. 6. Price. Provide pricing or a published price list outlining : a. Daily technician charges, include per diem for travel. b. equipment mobilization, c. equipment/supplies d. Option year increases should be stated as a percent increase. Technical evaluation of equipment and personnel based on the information provided and past performance references, when combined, are significantly more important than price in determining the best value source selection. The Government intends to award without discussion unless it is determined to be necessary by the Contracting Officer.
