Amendment # 2 Changes the Date Questions Are Due From: 3 April 2009 at 9:00 a.m. Mountain Standard Time To: 9 April 2009 at 12:00 p.m. Mountain Standard Time All questions must be submitted in writing to Contract Specialist Mary Hudson at Mary.L.Hudson@us.army.mil Amendment # 2 Changes the Statement of Work From: The Statement of Work, Dated: 23 March 2009 To: Statement of Work For MFF Training [High-Altitude-Low-Opening (HALO) Proficiency Training] Dated 7 April 2009 I. Background This program is a one stop training and sustainment package allowing Special Forces Groups the ability to train and sustain a Level 1 HALO/HAHO free fall capability on a regularly scheduled basis. This program is intended to sustain HALO/HAHO proficiency prior to combat operations. II. Specific Requirements 2.1 Aircraft: Skyvan SC7 aircraft, or aircraft of comparable capability, and aircrew to support Military Free Fall (MFF) High Altitude Low Opening (HALO) Proficiency Training. 2.2 The aircraft must be capable of in-flight operations, preferably ramped, and capable of four (4) lifts minimum with five (5) lifts preferred per day to an altitude not less than 13,000 feet MSL and not to exceed an altitude of 25,000 feet MSL. 2.3 Jump lights (red/green) must be in the cargo compartment for jumpmaster/aircrew coordination. 2.4 Seats must be side-facing seats (military style canvas/nylon seats with extended length belts) for up to sixteen (16) combat equipped jumpers per lift. 2.5 The aircraft must be configured for static line operations. Static line cables must be permanently installed; temporary installations are not acceptable. 2.6 There must be intercom communications between aircrew and jumpmaster. 2.7 Cargo compartments must be able to accommodate jumpers, instructors, safeties, and portable oxygen console (Skyvan SC7 minimum size). 2.8 Aircrew shall have oxygen systems (portable or permanent). 2.9 Standard VHF/AM radios are required with UHF radio preferred, if available. Aircraft must be configured for day and night HALO operations. 2.10 The contractor shall be DoD approved commercial operators to include aircraft certifications and licenses (Part 135 certified in accordance with DoD 4500.53) if available, or otherwise in accordance with USSOCOM Directive 350-8. 2.11 Aircraft should be able to read winds during climb and at exit altitude. 2.12 The aircrew shall be knowledgeable in current MFF tactics and procedures relating to aircrew and jumpmaster interface/coordination. 2.13 Daily usage will vary according to training profile requirements, averaging one and one-half (1.5) hours per day per aircraft. 2.14 Jumps will be scheduled for a thirteen day period. 2.15 The cost of the contractors lodging and per diem as well as the cost of transporting aircraft and aircrew to the performance site shall be included in the firm fixed price. 2.16 All aircrew shall be USA citizens with citizenship verifiable by provision of birth record or passport. 2.17 The contractor shall have insurance coverage to include parachute and HALO operations per DoD certification. 2.18 Training sessions will take place with variable hours during which time the contractor shall have the aircraft ready for operation. 2.19 Training location must have the Airfield, dropzone and Vertical Wind Tunnel within 1 Mile radius of each other. In addition contractor must have access to a Military surveyed drop zone capable of conducting Blind Drop operations that is no closer than 10 miles from a airfield with active lights and no further than 50 miles of an airfield with active lights. Drop Zone must be located in a geologically similar terrain(desert) to what is found in Iraq or Afganistan. Facility must include a Vertical Wind Tunnel coach, who must be familiar with Military Free Fall Course curriculum, airborne operations, policies and procedures. 2.20 Vertical Wind Tunnel Rental: The vertical wind tunnel must be 14 diameter chamber or greater within a 1 mile radius of Drop Zone, and Airfield. Facility must be able to support personnel and equipment up to four hundred (400) pounds. Facility must include a Vertical Wind Tunnel coach, who must be familiar with Military Free Fall Course curriculum, airborne operations, policies and procedures. Quantity of 8 Hours in the Vertical Wind Tunnel. Course hours will vary. 2.21 Instructor is responsible for providing all audio visual equipment to conduct training. 2.22 Company and Location: Airfield, Vertical Wind Tunnel and Drop Zone Facilities must be located within two miles of a facility capable of providing billeting for 36 personnel. 2.23 Period of instruction will be the first 3 days of training exercise of the below schedule. 2.24 Flight Hours: Total Flight Hours 25 Hours 2.25 Number of Personnel to Attend Training: 27 Personnel 2.26 Period of Performance will commence 20 April 2009 to 2 May 2009 Definitions Used: VWT-Vertical Wind Tunnel H-HALO (High Altitude, Low Opening) S-Standoff (High Altitude, High Opening) N-Night CE-Combat Equipment O-Supplemental Oxygen A-Administrative / Training NT-Non Tactical NAVAID-Navigational Aid (Compass Board, GPS, Lensatic Compass) PDB-Parachutists Drop Bag 13 Day Training Schedule: DAY 1: Objective: All training emphasis will be focused on canopy control skills. Quality Assurance Surveillance Plan 1. Purpose: This Quality Assurance Surveillance Plan (QASP) describes methods that will be used to monitor the Contractor's performance. The QASP is not part of the Contract it is provided to the Contractor solely' for information purposes and is subject to change by the Government during the life of the contract. The Government reserves the right to perform assessments more or less often than outlines in this QASP. 2. Introduction: The QASP establishes methods to assess or evaluate performance of the contractor under the contract. It is designed to provide an effective method of monitoring Contractor Performance. The Government is concerned with the quality of the services provided, and not with the method that the Contractor uses to provide the services. 3. Responsibility for Quality Control: The Contractor is responsible for quality control and management actions to meet the performance requirements and terms of the contract. The Government performs quality assurance to assess the quality of services rendered. The services shall meet the contract requirements, and ensure the contract standards are achieved. 4. Surveillance Methods: The Government will evaluate the Contractors performance under this contract. Examples of Government surveillance may include: a. Government Inspections b. Review of Contractor's Quality Control documentation 5. Deduction Methods: The Government will record performance noting any substandard performance. For substandard performance, the Government's preferred course of action will be discussions with the contractor to make the Contractor aware of the problem, and to effectively prevent any recurrence of substandard performance. The Government may also take the following actions: a. Require correction or re-performance of the service b. Presentation of a Contract Discrepancy Report requiring a written explanation of substandard performance c. Payment deductions in accordance with applicable FAR c1ause(s). IV. Contractor Manpower Reporting The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address https://contractormanpower.army.pentagon.mil. The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officers Technical Representative; (2) Contract number, including task and delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Estimated direct labor hours (including sub-contractors); (6) Estimated direct labor dollars paid this reporting period (including sub-contractors); (7) Total payments (including sub-contractors); (8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if different); (9) Estimated data collection cost; (10) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information); (11) Locations where contractor and sub-contractors perform the work (specified by zip code in the United States and nearest city, country, when in an overseas location, using standardized nomenclature provided on website); (12) Presence of deployment or contingency contract language; and (13) Number of contractor and sub-contractor employees deployed in theater this reporting period (by country). As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending September 30 of each Government fiscal year and must be reported by 31 October of each calendar year. Contractors may use a direct XML data transfer to the database server or fill in the fields on the website. The XML direct transfer is a format for transferring files from a contractors system to the secure web site without the need for separate data entries for each required data element at the web site. The specific formats for the XML direct transfer may be downloaded from the web site.
