- ATTACHMENT L.7: PRICE TEMPLATE ATTACHMENT L.6: PAST PERFORMANCE CONTRACT SUMMARY TABLE ATTACHMENT L.5: PAST PERFORMANCE QUESTIONNAIRES ATTACHMENT L.4: VOLUME IV, PRICE CROSS REFERENCE TABLE ATTACHMENT L.3: VOLUME III, PAST PERFORMANCE CROSS REFERENCE TABLE ATTACHMENT L.2: VOLUME II, MANAGEMENT CROSS REFERENCE TABLE ATTACHMENT L.1: VOLUME I, BUSINESS CROSS REFERENCE TABLE FINAL SOLICITATION DOCUMENT - SF-33 AMENDMENT 002 - APRIL 10, 2009: ISSUANCE OF FINAL SOLICITATION The purpose of this Amendment 002 is to issue the Final Solicitation for the THU Support Services acquisition. Proposals are due May 11, 2009, 12pm EST. All other proposal submission instructions are contained in the SF-33, solicitation document and attachments. Offerors must submit all questions concerning this solicitation in writing to the Contracting Officer and Contract Specialist. They must be received no later than ten (10) calendar days after the date of this solicitation. All responses to the questions will be made in writing and included in an amendment to the solicitation. The Contracting Officer (CO), Ms. Carolyn Abney, may be reached at the carolyn.abney@dhs.gov and the Contract Specialist (CS), Ms. Mary Petruzzo may be reached at mary.petruzzo@dhs.gov. All inquiries shall be submitted by email as a Microsoft Word attachment to the above e-mail addresses. Emails must state in the subject line: “THU Support Services RFP Questions.” Offerors must identify the applicable RFP Section, page number, and paragraph number for each question. Only written responses from the CO incorporated through an RFP amendment shall be considered binding for this procurement. The Government will publish RFP revisions resulting from potential Offeror questions by Amendment(s) to the RFP on FedBizOpps. It is each Offeror’s responsibility to monitor FedBizOpps for Amendments to the RFP. All Amendments shall be acknowledged in Block 14 of the Standard Form 33 as part of the proposal submission. Only prospective prime Offerors shall contact the CO and only the CO in regard to this solicitation prior to award. Prospective prime Offerors are cautioned to advise potential Subcontractors of this instruction. If contact with any other Government or Contractor support personnel occurs and is found to be prejudicial to competing Offerors, the Offeror (including Subcontractors making such contact) may be excluded from award consideration.
