- SF 330 Architect-Engineer Qualifications applications are invited from Architect-Engineering firms to provide a professional survey of penetrations in smoke and fire walls in buildings that house overnight patients at the American Lake and Seattle Divisions of the VA Puget Sound Health Care System, 9600 Veterans Drive, Tacoma, WA 98493-5000. The survey work will involve approximately 380,000 gross square feet in the existing Bldg 81, 7, 6, 4, and 5 at the American Lake Division and Building 100 at the Seattle Division. The proposed survey project is for the inspection, labeling / recording, and remedial recommendations of penetrations of smoke and fire barrier walls in 6 patient (overnight) occupied buildings at the Seattle and American Lake Divisions of the VA Puget Sound Health Care System. The survey work will involve extensive on-site work with 100% physical inspection of defined smoke and fire walls in the specified buildings. This project will require planning coordination between the VA engineering service and professional survey firm to develop a coordinated identification system for each penetration. The contract deliverables will be coordinated penetration labeling, spreadsheet and building floor plans showing all penetrations. The contract survey work and recommendations must be performed in accordance with American Society for Testing and Materials E 814; ASTM E 1966; ASTM E 2174 and ASTM E 2393. The time of delivery of the final survey report is 120 calendar days following survey award. Evaluation and selection will be based on qualifications stated under CBD Numbered Note 24. The selected firm or organization must have significant experience in the ASTM fire surveys serving institutional health care buildings. Applicants must be able to demonstrate the credentials of the firm and assigned staff to support the survey scope. The applicable NAICS code is 541380. This is not a Request for Proposal. A current SF 330 representing and differentiating between the personnel, experience and capabilities of all involved offices, must be included with the application for each proposed consultant's office involved.
