- 17. Reference item number 10; Closing Response date is as stated above or 30 days after release of the solicitation which ever is later. Reference previous sources sought synopsis dated Dec 2007, May 2008 and pre-solicitation notice dated 10 Feb 2009. This office intends to issue a Request for Proposal (RFP) number FA8900-09-R-0002 for the 711 Human Performance Wing (HPW) Mission Support Services sometime in May 2009. The 711 HPW is a geographically dispersed Wing headquartered at Wright-Patterson AFB (WPAFB), Ohio, with a significant mission operating the USAF School of Aerospace Medicine (USAFSAM) at Brooks City-Base (BCB), Texas. The unit is mandated under the Base Realignment and Closure law to complete its mission transfer from BCB to WPAFB by the end of FY11; under a reorganization accomplished in FY08, the USAFSAM absorbed all the responsibilities of the Air Force Institute for Operational Health (AFIOH). The merger of AFIOH into USAFSAM resulted in a diverse portfolio of large and small business contracts, many of short duration and/or due to expire. All the requirements capable of being accomplished by 8(a) small business companies have been consolidated under this 8a set-aside procurement. This 8(a) set-aside will result in multiple (3 or more) indefinite delivery, indefinite-quantity contract awards. This acquisition will provide a full range of Advisory and Assistance Services (A&AS), Non-A&AS, and Personal Services to the USAF School of Aerospace Medicine in its acquisition of medical, clinical, biological sciences support for the HPW. Specific requirements will be met by issuing task orders under the basic contracts. The contract types for these task orders are expected to be performed via Firm-Fixed Price (FFP) and Time and Materials. The ordering period for the basic contracts will be five years and the total ceiling amount for all basic contracts will be $93 million. For the contract duration, the minimum guarantee amount shall be $55,000. This minimum guarantee shall be met by obligating $55,000.00 or more via the first task order. Task Order 0001 will be issued concurrently with the basic contract award and is not included in the basic contract proposal costs. The RFPs associated with the first task orders will be issued along with the notice of awards. Those services considered most critical to mission success are as follows: Information Technology personnel Biomedical Laboratory Technicians Molecular Biologists Medical Specialists Ophthalmologists Psychologists Physicians Neurologists Toxicologists Cardiologists Anticipated award date for the basic ID/IQ contracts is 18December 2009. Until such time, contacts or briefings by participating offerors concerning this acquisition will not be allowed outside the formal source selection process. The Contracting Officer (C) shall control all contact or exchanges with offerors and is the only person authorized to release information before and after contract award. Therefore, all communications regarding this competitive 8a Set-Aside acquisition shall be through the Contracting Officer only. In accordance with the Draft RFP Section L, page 5, Paragraph 2.1.3 Discrepancies, "if and Offeror believes that the requirements in these instructions contain an error, omission, or are otherwise unsound, the Offeror shall submit questions/concerns to the Contracting Office with supporting rationale immediately, but no later than 10 calendar days after release of the RFP. Note: if any potential offeror has any questions/concerns regarding the current Draft RFP documentation, please submit it as soon as possible but no later 17 April 2009 to the Contracting Officer. The current draft documents attached to the previous announcements on FedBizOpps are not a solicitation. However, upon release of the RFP these documents will be in final form. The Government request that Offerors submit Past Performance Information (PPI) forms for each relevant project to be evaluated for Past Performance no earlier than the final RFP release date and at least 15 calendar days prior to the date set for receipt of proposals. Offeror references can use this time to complete their associated Past Performance Questionnaire (PPQ). Failure to submit early PPIs will not necessarily result in Offeror disqualification. Pre-Award Audit Registration Request: After release of the final RFP, the Offeror, to include each firm of a Joint Venture and if applicable, any new accounting systems created for the Joint Venture, shall complete the Pre-Award Audit Registration Request and submit to the Contracting Officer to initiate, if needed, an accounting system audit by DCAA. The following information will be required (Prime only): 1) Prime's Full Company name; 2) Prime point of contact; 3) Prime POC phone number; 4) Address where accounting records are held; 5) Cognizant DCAA office-region and branch. This request should be accomplished immediately upon receipt of the RFP but no later than 15 calendar days after release of the RFP. Failure to submit early proposal information will not result in Offeror disqualification. Financial Capability Request: After release of the final RFP, the Offeror shall submit financial capability information (for self and all team members) immediately upon receipt of the RFP but no later than 15 calendar days after release of the RFP. The following information will be required: 1(Full Company's Name; 2) Company's point of contact name; 3) Company's POC phone number; 4) Address where financial records are held; 5) Cognizant DCAA office-region and branch; and 6) submit the last three (3) years of financial statements and year-to-date financial information through the last quarter available for prime and teaming partners.
