- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. This solicitation number is W912NR-09-R-0008 is a Request For Proposal (RFP) and is set aside for Small Business. Small businesses are encouraged to respond under NAICS 621511,621399, and 621999. The RFP incorporates provisions and clauses as those in effect through Federal Acquisition Circular Number 2005-15 and 2005-29 and Defense Acquisition Circular Number 91-11, 20081124 Edition. This RFP is being conducted as a commercial service procurement using Simplified Acquisition Procedures (FAR Part 13). No paper copies of this solicitation will be provided. This solicitation is set aside for small business; however, large business will be considered if no small businesses respond with a proposal. Contract financing will NOT be provided for this acquisition. The Government reserves the right to not make an award at all. All interested vendors shall provide this office, by email, a proposal no later than 10:00 a.m. on 8 May 2009 C.S. T. No facsimile or hard copy responses will be honored. Brief Description of Requirements: The Louisiana Army National Guard has a requirement to provide medical surveillance examinations to its full-time Technician workforce as specified by Statutory Law 29 CFR 1910 OSHA General Industry Regulations. The estimated number of examinations (270) will be required in the following Louisiana cities: Pineville (130), Fort Polk (85), Hammond (35), and Slidell (20). The anticipated length of this contract will exist no earlier than 1 June 2009 through 30 September 2009. All exams, procedures and tests will be performed by properly State of Louisiana licensed health care providers. An occupational medical surveillance will consist of a physical examination to include: 1) complete occupational health history, 2) physical examination, 3) anthropometric measurements, 4) vital signs, 5) complete blood count with differential, 6) blood chemistry profile, 7) urinalysis, 8) audiometric testing, and 9) stool for occult blood. Based on age and occupational exposures, additional testing will include: a) audiometric testing only, b) PSA, c) electrocardiogram (12 lead with interpretation), d) blood lead and ZPP (zinc protoporphyrin) level sampling and analysis, and e) pulmonary function testing (FVC, FEV1, and FEF 25-75). Provide charges for unit costs to include daily equipment, labor, and administration fees. Setup and travel fees, daily equipment usage fees, daily labor fees (technicians) daily labor fees (technician lab), physician labor fee (hour rate). Exam fees: work & medical history, PA Chest-x-ray, pulmonary function profiles, audiometric examination, visually acuity, cardiogram, blood pressure/height-weight, Lab Analysis-Lab Profile #P2103 Chem Screen 25/CBC/Urinalysis, Lab Profile OSHA Blood Lead/ZPP, Respirator Fit test Quantitative, Physical Examination Workplace Clearance Respirator Usage, & Data Processing & Record Management Fees. The following clauses and provisions are applicable to this requirement. 52.212-1 Instructions to Offerors - Commercial Items. 52.217-5 Evaluations of Options. 52.217-8 Option to Extend Services. 52.217-9 Option to Extend the Term of the Contract. 52. 212-2 Evaluation - Commercial Items, factors are as follows: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: The offeror shall provide the following information for source selection. 1. Price. Provide pricing or a published price list outlining: a. Daily technician charges, include per diem for travel. b. equipment mobilization, c. equipment/supplies. 2. Information concerning the company offering the proposal 2. Technical information concerning mobile equipment to be used to fulfill the contract. 3. Resumes of personnel (health professionals and support staff) key personnel intended to be assigned to work on this effort. 4. Past Performance references: Include the name of the agency/company, telephone number, name of an individual that has knowledge of your past performance. Price is significantly more important than technical evaluation of equipment and personnel based on the information provided and past performance references in determining the best value source selection. The Government intends to award without discussion unless it is determined to be necessary by the Contracting Officer.
