- The Uniformed Services University of the Health Sciences (USUHS), the DoD Medical School and Health Sciences University, Bethesda, intends to procure, on a competitive basis, preventative maintenance service for the following: •1) Model 81-10 Calibrator, Serial #: 7055 •2) Model 81-18 Calibrator, Serial #: 7151 A combined synopsis/solicitation for commercial items and services is prepared in accordance with the procedures in Subpart 12.6, as supplemented with additional information included in this notice. This solicitation will be issued as a request for quote (RFQ). USUHS intends to award an option year purchase order with a base period of twelve (12) months with four (4) one year options. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular (FAR) 05-26. The North American Industry Classification System (NAICS) code is 334517 ( Irradiation Apparatus Manufacturing ) and a small business size standard of 500 employees. It will be the responsibility of the contractor to check FedBizOpps for any amendments to the solicitation. Evaluation Factors : The government will award a firm-fixed-price (FFP) contract or contracts resulting from this solicitation to the responsible offeror whose quote, conforming to this solicitation; will be most advantageous to the government, considering price and other related factors to include demonstrated experience in providing this service, and past performance. Award will be made on a best value determination considering price, demonstrated experience, and past performance. Please submit all questions and concerns in writing by April 20, 2009 at 4:30 PM EST. All questions must be directed by email to Karen Mulzac at karen.mulzac@usuhs.mil. All quotes are due on April 27, 2009 at 1:00 PM EST. All quotes must be valid until May 30, 2009. Anticipated award date is May 1, 2009. All quotes from responsible sources will be considered. (d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement.
