Picture of the Medal on the base of the Outstanding Graduate Award Trophy Picture of the Outstanding Graduate Award This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. SOLICITATION #: F3G5GA9057A001 This combined synopsis/solicitation is issued as a Request for Quotes NAICS: 423940 This combined synopsis/solicitation is being issued as 100% Small Business Set-Aside. SBA Size Standard: 100 employees Nellis AFB, Las Vegas, NV requires a contract for: CLIN 0001: Graduate Trophy Quantity: 37 Each Description: Small Pilot Trophy, USAF Weapon School Trophy Graduate Award, Standard Trophy Part Number: G4P-2 (Line Group, Inc) "OR EQUAL" Walnut base with male fighter pilot on top; brushed gold (brass) engraving plate. Dimensions: Approximate overall height 12 inches, Base: Approximately 5x5 inches CLIN 0002: Outstanding Graduate Trophy Quantity: 22 Each Description: Outstanding Graduate Award, Standard Trophy Part Number: G4P-1 (Line Group) "OR EQUAL" 4 Poster walnut base with lid, male fighter pilot on top, 4 bronze eagle trims, brick wood risers and brushed gold (brass) engraving plate Circular medallion affixed on base within the middle of the four posts to reflect an Eagle and leaves design. The following shall be written on the medallion: "USAFWS Outstanding Graduate Award" Dimensions: Approximate overall height is 20 inches, Base: approximately 9x9 inches (Reference Attached picture) CLIN 0003: Best Paper Plaque (8x10 inches) Quantity: 1 Each Description: premium walnut plaque, 8x10 inches, with full color and engraved plate. (Reference Attached picture) The requested plaque shall have the same blue and white background as the plaque in the picture and shall have the patch shown in the attachment titled "USAFWS Patch". The design plate shall be approximately 4x6 inches. The Engraving Plate shall be approximately 2x4 inches. Example of the engraving: BEST PAPER USAF Weapons School Class 09A Capt John C. Doe Additional Requirement: Information for completing engraving plates shall be faxed or emailed to the contractor 1 week prior to the delivery date. The trophies must be delivered on 12 Jun 09 (exact date) at Flamingo hotel in Las Vegas, NV. Contractors/Vendors shall provide a telephone contact number and an email address to arrange delivery. Contractors/Vendors shall be available on Saturday 13 Jun 2009 for any corrections that must be made to the engraved plates, and upon telephone notification must deliver the corrected engraving plate to the Flamingo Hotel as required prior to 3:00 pm PDT on 13 Jun 2009. Any questions asked after the specified date and time shall not be answered. This is a firm fixed price type contract.
