PROPOSED FED BIZ OPS ANNOUNCEMENT General Information Document type:Special Notice Solicitation Number:W912K6-09-Q-1001 Posted Date:Apr 17, 2009 Original Response Date:Apr 24, 2009 Current Response Date:Apr 24, 2009 Original Archive Date:May 23, 2009 Current Archive Date:May 23, 2009 Classification Code:Q Medical Services NAICS Code:621111 Contracting Office Address USP&FO For Maryland, Purchasing & Contracting Division, 301 Old Bay Lane, State Military Reservation, Havre de Grace, Maryland 21078-4094 Description NOTICE OF INTENT TO AWARD SOLE SOURCE: The United States Property & Fiscal Office for Maryland intends to negotiate a firm fixed price contract, on a sole source basis, with Chesapeake Occupational Health Services, 11332 Red Lion Road, White Marsh, Maryland 21162 for the purchase of mobile occupational health examinations/services, one hundred eighty-eight (188) each. Sole source is based upon the contractor being the only available regional source that can provide the number of required mobile examinations at specific geographic locations. Examinations must be complete prior to training deployment of military personnel. Types of examinations required are Type A (Occupational Health) examination which includes Medical History, Physical Exam, Urinalysis, Lab Work (Comprehensive Metabolic Panel, Lipid Profile, CBC with differentials, PSA for male personnel over 40), Vision Screening and Audiogram; Type R (Respirator) Examination includes Medical History, Physical Exam, Pulmonary Function Test, Audiogram, Respiratory Clearance, Urinalysis, same Lab work as Type A Examination & Vision Screen; Type W/P (Welder/Painter) Examination includes Medical History, Physical Exam, Function Test, Audiogram, Respiratory Clearance, Urinalysis, Lab Work same as Type A Examination plus Lead ZZP/FEP, Vision Screening and Chest X-Ray. Location of exams are Edgewood, Reisterstown, Laurel and Havre de Grace, Maryland. In the absence of any additional qualified sources, a contract will be awarded under the authority of FAR 13.106-1(b) which allows for sole source purchase if it is considered an unusual and compelling urgency. Interested concerns may identify their interest and capability to respond to the requirement or submit proposals. Solicitation documents are not available. This notice of intent is not a request for competitive proposals. A determination on whether to conduct a competitive procurement, based upon any responses to this notice, is solely within the discretion of the Government.
