- Technical Evaluation Requirements and Proposal Instructions Statement of Work The Centers for Disease Control and Prevention has a requirement to develop a state-of-the-art, customized, virtual reality training module (Phase II), that will integrate seamlessly with the current module, that enables deployable staff in this special training program to safely experience many of the potentially-stressful sights, sounds and smells they could experience in the field if they respond to either a domestic or international public health emergency. Base Year and Three Optional Years begin on June 2, 2009. Offerors must propose in accordance with the attachment to this solicitation DUE DATES FOR QUESTIONS AND PROPOSALS: Questions are due NLT 5:00 p.m., Eastern Standard Time April 30, 2009 Responses to question will be sent to all offerors NLT May 6, 2009 Quotations are May 14, 2009 NLT 5:00 p.m., Eastern Standard Time The Government intends to evaluate offers and award a contract without discussions and Offerors providing the requested service. Therefore, the initial offer should contain the offeror's best terms form a technical and price standpoint. Source Selection and Award will be made to the offeror whose proposal is consider to be the best value and most advantageous to the Government. Technical evaluation factors will be more important than cost/price in evaluation of quotations. The North American Industry Classification (NAICS) code is 541511 and the Size Standard is 500 employees for this requirement. All organizations may submit a proposal for this Request for Quotation (RFQ). However, the Government will evaluate proposals only from organizations that are certified by the U.S. Small Business Administration as small business concerns. Each offeror needs to identify and certify its size status in the Representations and Certifications submitted with the quotation. Interested vendors capable of furnishing the Government with the services in this synopsis and Statement of Work attached should submit quotations to the email address listed below. The quotation shall include the itemized price for base year and three option years, prompt payment terms, the F.O.B Point (Destination or Origin). Taxpayer Identification Number (TIN), Dun & Bradstreet Number (DUNs), and descriptive literature of the product quoted. The quotation must reference Solicitation Number 2009-Q-11276. Quotations will be due on May 14, 2009 by 5 pm Eastern Standard Time. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted via email to Lrobinson@cdc.gov. Any interested party that believes it can meet the above requirements, should submit a quotation. The quotation and any other furnished information must be in writing and must contain material in sufficient detail to allow CDC to determine if the party can fully meet the requirements herein. The requirement will be procured as a commercial item in accordance with FAR Part 12.102 ( c ). THIS IS A COMBINED SYNOPSIS/SOLICATION FOR COMMERICAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The following provisions apply to this acquisition; 52.212-1 Instructions to Offerors-Commericals items (APR 2008); 52.212-2, Evaluation-Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical approach to managing and coordinating multiple task as described in the atatement of work; key personnel and management plan, (2) price, and (3) past performance. The clause 52.212-4, Contract Terms and Conditions - Commercial Items (FEB 2007) applies to this acquisition. The following FAR Clauses are hereby added to this clause: 42.247-35 F.O.B. Destination; The clause at 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders - Commercial Items (MAY 2008) applies to this acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquisition: (1), (3), (5), (7), (9), (11), (12), (14), (15), (17), (18), (20) and (24). OFFERORS SHALL INCLUDE A COMPLETED COPY OF THE PROVISION AT 52.212-3, OFFERORS REPRESENTATION AND CERTIFICATIONS - COMMERICAL ITEMS (NOV 2006), WITH YOUR PROPOSAL.
