- 1449 with clauses and provisions This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being rquested and a written solicitation will not be issued. STATEMENT OF WORK - TEMPORARY NON-PERSONAL PHYSICIAN SERVICES A. SUBJECT: The provision of professional non-personal physician services, by a licensed medical physician, for the male inmate population at the United States Penitentiary - Lee and Federal Prison Camp - Lee, Jonesville, Virginia. B. PLACE OF PERFORMANCE: Services will be provided at USP and FPC - Lee, Jonesville, Virginia. C. SCHEDULE: The scheduling will consist of not more than 30 hours per week (six to eight hours per day), Monday through Friday, excluding weekends and federal holidays. A mutually agreed upon time and day will be established. D. SUPPLIES: All supplies the contractor may need while at the institution will be furnished by USP Lee. If specific equipment/supplies are required by the contractor, he/she shall make the need known to the appropriate personnel. All records remain the property of USP Lee. E. METHOD OF PERFORMANCE: The services provided, being professional in nature, will be provided in a method considered normal by community standards. These services, although not directly supervised, will be reviewed by the Health Services Administrator at USP Lee. The performance of these services will also be verified by the delegated contract monitor. F. DESCRIPTION OF RESPONSIBILITIES: The contractor will be responsible for performing the following clinical functions: 1. Examine, diagnose, and treatment of medical conditions with appropriate referral, if necessary, to a specialist. 2. Order x-rays, laboratory tests, and other tests necessary for diagnoses. 3. Prescribe pharmaceutical drugs that are listed in the approved BOP formulary. 4. Evaluate referrals from clinical staff, including physician assistants and nurses, providing physician services which conform to community standards and all local, state, and federal laws and regulations applicable to the delivery of health care to members of the general public. 5. Observe and adhere to institutional policies and procedures established to facilitate delivery of health care to the inmate population at USP and FPC Lee. G. INMATE MANAGEMENT: The contractor will not be responsible for the management of inmates. H. INSTITUTION: The contractor agrees to adhere to all regulations prescribed by the institution for safety, custody, and conduct of inmates. The contractor will be required to attend a four hour contractor orientation class, to assure that he/she understands the policies and procedures. This orientation is in addition to an orientation provided by a member of the Health Services staff. An identification card (contractor pass) will be issued and worn inside the institution for proper identification. The contractor will be required to sign in an out of the institution for accountability purposes. I. QUALIFICATIONS: Respondent must meet the following qualifications: 1. Valid and current registration as a professional physician, either M.D. or D.O. in the Commonwealth of Virginia. 2. Minimum of three years recent experience is acute care, ambulatory care, medical/surgical or psychiatric care facility. Experience in a correctional health care setting preferred, but not required. 3. Proof of CPR certification. 4. ACLS certification preferred. 5. Evidence of recent testing for or immunity to tuberculosis or other communicable diseases. J. TRANSFER OF ASSIGNMENT: Neither this contract nor any interest herein or any claim arising hereunder may be transferred or assigned by this contractor or any other individual or agency. K. COVENANT AGAINST CONTINGENT FEES: The contractor warrants that he/she has not employed any person to solicit or secure this contract upon any agreement for commission, percentage or contingent fee. Breach of this warranty shall give the Government the right to annul this contract, or, at its discretion, to deduct from any compensation or other amount payable hereunder, the amount of such commission, percentage, or contingent fee. L. PERIOD OF CONTRACT: Period of contract will be from May 14, 2009, through September 30, 2009.
