Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W67K2S-9061-0500 is being issued as a Request for Quotation. This solicitation incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2001-12. This will be issued as Full and Open Competition, the North American Industry Classifications System Code (NAICS) is 621111. SPECIFICATIONS: The USPFO for Utah has a requirement to contract for Respiratory Fit and Heath Testing services for the Utah National Guard at two designated locations. Health screen tests shall include the following: 1.Respiratory fit testing IAW CFR 1910.133. Contractor shall have the capabilities to fit test North 7500, 3M 7800 and Survive Air respirators. Contractor shall provide initial respiratory fit tests and appropriate size for new specified personnel. Utah Army National Guard will provide a list of new employees and adaptors for respirators. Tests shall be reviewed and signed off by an OH board certified physician. 2.Vision testing IAW CFR 1910.134, PFT, Height, Weight and Blood Pressure Testing. Records and test results shall be reviewed with an OH board certified physician with notification given on any employees that require follow-up evaluations. 3.Proposed Health Provider shall be willing to work with the occupational health nurse (LTC Marcus) to ensure the delivered product is in a format compatible with the Defense Occupational Health Readiness System Hearing Conservation (DOHRS-HC) so data can be downloaded and forwarded to Ft. Detrick. 4.Contractor is responsible to furnish all personnel and equipment necessary to provide testing. The Government will provide suitable space, electrical and water connections. It is the anticipation at this time to award a Base Year contract and to include Four additional option years. Submit your quote referencing each option year requested, i.e., Base Year, Option Year 1, Option Year 2, Option Year 3 & Option Year 4. Selection will be based on the following evaluation factors, in order of importance: (1) Offerors capability to provide a quality product that meets the Governments needs; (2) Past performance, caliber of offerors performance on previous contracts of a similar nature; and (3) Total cost or price. Note also that Technical and Past Performance, when combined, are significantly more important than cost. However, if competing Companies are fully qualified, price will become the deciding factor. ADDITIONAL INFORMATION: In accordance with FAR 52.212-1(k), prospective awardees shall be registered and active in the CCR database prior to award. Offerors may obtain information on registration at www.ccr.gov. And also in accordance with FAR 52.212-3, Offeror Representations and Certifications Commercial Items, Offerors shall have completed registration on the ORCA website at https://orca.bpn.gov.
