23 Apr 09 Site Visit Remodel Extended Care Unit (Bldg 88) Project 531-08-115A pre-proposal site visit was conducted on 23 April 2009 at the Boise VA Medical Center. The following individuals were in attendance; Mike Willis-Contracting Officer, Joel Holtrop-Engineer, Bryan Lightfield-Engineer, Brain Davis-Gulf Tech, Brian Donahue-PMG, Joe Paige-PMG, Jim Holm-Northwest Technologies, Tim Foley-FMG, Brain Schneider-Ridgeway Industrial, Jack Daniels-Northcon, Inc, Larry Landa- Larkor, Sandi Torvias-Larkor, Bill Welch-CMEC, Inc, Steve Jacobsen-Construction Contracting Group, Mark Howard-Reborn Construction, Brian Cornell-Rundle Electric, Larry Higginson-Burton Construction, Chris Schowalter- Sygnos Inc, Antonio Carowe-Syngos Inc., David Stauffer-Alpine Construction. Questions and Answers 1.Are the entrance mats recessed? Yes there are three areas identified on sheet A-111 at the end of corridors (C11, C7, C3) calling for installation of recessed walk off mats/carpet. This will require the current slab on grade floor to be modified accordingly at these three locations 2.Will the project be phased? Yes. The project site is a 24hr residential facility. Work will be required to be phased in a minimum of 5 phases to ensure uninterrupted operations and patient care. As each phase is completed it will be returned to the customer for beneficial occupancy and the subsequent phase will begin. Final punch list to be completed at the end of the project. 3.What are the work hours? Work is expected to be completed during weekday business hours of 8:00 AM until 4:30 PM. 4.Does the scope of work include the corridors? Yes. The project description on sheet A-111 identifies six specific work areas in addition to the corridors. The finish and plans schedule on Sheet I-1.01 identifies the corridor work area as well. 5.Are all the ceiling panels /tiles to be replaced? No, the intent is for the Contractor to provide tile to match existing and replace tiles as necessary (up to 40) to ensure a uniform ceiling appearance throughout the project 6.Who is responsible for removing and reinstalling items off the corridor and rooms walls? See General Notes provide with solicitation package. 7.Who is responsible for storing furniture and equipment items during work? The VA will provide the contractor a designated area for storage. 8.Will fire sprinkler heads be relocated as part of this project? No. 9.Is there asbestos in this facility? No. The facility was constructed in 1985. 10.Will the contractor be responsible for temporarily relocating the communication and IT support for the nursing station? No. The VA will coordinate and establish a temporary nursing station. 11.Will the handrails in the corridor be replaced? No. However, for painting purposes, the Contractor may remove and reinstall. As part of the installation of the new hand washing stations the handrails will need to be modified accordingly. (Sheet A-112, Note J). 12.Will the wall covering need to be removed? Yes. All wall covering in the work areas and corridors will need to be removed and replaced with the appropriate finish as designated in the finish schedule. 13.Will diffusers need to be relocated? Yes. Please see sheet M-101, Note 1 and sheet M-102, note 5. 14.Do we need to demo the accordion door track in work Area 6? The existing track will not be reused. See Sheet A-112, Note 10. Contractor will provide new drapery and track (Note R) per finish room schedule. As a clarification to Note 7, on Sheet A-113, strike out (remove track at contractors option)
