- Request for Quote (RFQ) 2009-Q-11285 Statement of Work - Effectiveness of Rotavirus Vaccine The Centers for Disease Control and Prevention (CDC), National Center for Immunizations and Respiratory Diseases (NCIRD), Division of Viral Disease is actively seeking a qualified health care organization or business to: 1. Assess the duration of pentavalent rotavirus vaccine protection among children 1 - 3 years of age. 2. Determine the vaccine effectiveness of mixed receipt of monovalent and pentavalent rotavirus vaccine among children 15 days - 12 months of age. 3. Evaluate vaccine effectiveness against the most commonly detected strain during the 2009 season. The vendor shall provide all labor, equipment, supplies, and facilities necessary to 1) conduct active surveillance in the emergency department and inpatient wards to identify children under three years of age presenting with acute gastroenteritis, 2) collect immunization information from all enrolled children, and 3) enroll control patients under three years of age presenting with acute respiratory illness. All collected data including clinical histories of diarrhea patients, rotavirus test results, and immunization records from all cases and controls- to include a brief report with information deemed to be relevant for data analysis and interpretation - shall be delivered to the appointed CDC Project Officer. Additionally, aliquots of serum, plasma, and stool specimens shall be sent to the viral gastroenteritis laboratory at CDC. Data shall be submitted in Access, Excel, or other similar flat file format. The period of performance will not exceed one year or 12 consecutive months following award of a U.S. Government Purchase Order. SEE THE ATTACHED DOCUMENTS FOR FURTHER DETAILS. The Government intends to evaluate offers and award a Purchase Order (PO) utilizing simplified acquisition procedures in accordance with FAR Part 13, to exclude Subpart 13.5. A best value award will be made to the offeror of a technically acceptable proposal with the lowest price. Therefore, the initial offer should contain the offeror's best terms from a technical and price standpoint. Interested vendors capable of furnishing the government with the commercial services as defined in the attached Request for Quote (RFQ) should submit a copy of your quotation. The quotation must include the itemized price, prompt payment terms, Dun & Bradstreet Number (DUNs), descriptive literature of the services quoted, and the vendor's qualifications as defined in the Statement of Work. All responsible sources may submit a quote, and if the offer is received in a timely manner, it shall be considered for award by the agency. Quotes will be due May 04, 2009, by 5:00 PM Eastern Standard Time. Quotes must be submitted in writing via email or fax to the Centers for Disease Control and Prevention (CDC), Procurement and Grants Office, 2920 Brandywine Road, Atlanta, GA 30341, Attention: Russell T. Oxford. Quotes must be dated and signed by an authorized representative. Offerors are required to submit their quotes electronically (via e-mail) in MS Word or MS Excel files to roxford@cdc.gov.Facsimile quotes will be accepted at fax number: 770-488-2044. The Central Contracting Registry (CCR)( http://www.ccr.gov ) is a central repository of all Contractors who wish to do business with the Federal Government. All vendors who wish to bid on any U.S. Government requirement / solicitation MUST be registered and validated in the CCR prior to award of a U.S. Government Purchase Order. Vendors who are not registered will not be considered for selection or award.
