- Amendment 0003 OSHA Form 174 Section L Attachment 7 Amendment 0003 is hereby provided This notice provides offerors additional information regarding the Range Safety Clearance, the Site Visit and the mandatory Flight Demonstration as a result of the change in location from NAWCWD China Lake, CA to the Yuma Test Center (YTC), AZ. In addition this notice announces the issuance of Amendment 0003 to N00019-09-R-0226. With respect to the Range Safety Clearance, and as a result of the change in location, YTC requires submittal of the following information prior to receiving approval to operate on the YTC range: 1) submittal of Material Safety Data Sheet (MSDS) for the system to be demonstrated; and 2) submittal of an aircrew certification memorandum. MSDS. A blank MSDS form is included with this announcement. Offerors are instructed to submit the completed form to the YTC point of contact listed below no later than 27 May 2009. Aircrew Certification. All offerors shall submit a memorandum on company letter head identifying the certification of all aircrew personnel participating in the mandatory Flight Demonstration to the YTC point of contact listed below no later than 27 May 2009. Aircrew personnel are those personnel who have direct control over operation of the air vehicle and its mission systems, including payload. Examples of aircrew personnel include air vehicle operators, internal pilots, external pilots, and payload operators. The certification memorandum should be addressed to the Yuma Test Center, Code TEDT-YPY-AV and should state that aircrew personnel are certified in accordance with the offeror’s aircrew certification process. The following additional information shall be included for each member of the aircrew: 1) Total number of flight hours; 2) Number of flight hours in the previous 90 days; and 3) Date of last flight. Submit both MSDS and Aircrew Certification to Mary Beth Weaver, YTC Aviation and Air Delivery Systems Division at Email: Mary.Beth.Weaver@us.army.mil. Site Visit Update The STUAS/Tier II UAS program will conduct a Site Visit on Tuesday, 12 May 2009 at Yuma Proving Grounds (YPG). The purpose of the event is to provide offerors an opportunity to view the site for the mandatory Flight Demonstration. Attendance is at offeror’s own expense. Two attendees are permitted per offeror. Offerors are encouraged to bring food and plenty of water to the site visit. Agenda (subject to change): 0730 Arrival at Pass & ID 0730 - 0800 Visit request processing 0800 – 0830 Transit to Bldg 2887 (Offerors will be escorted) 0830 - 0930 Range Safety, Airspace, Groundspace briefings; mishap planning and hazardous material handling overview 0930 - 0945 Break 0945 - 1015 Transit to Flight Demonstration site (Site 8) 1015 - 1145 Guided tour of site 1145 - 1200 Departure (Offerors will be escorted back to Rt. 95) Demonstration Instruction. The RFP, Section L, Technical Attachment 7 entitled “Instructions for the STUAS/Tier II UAS Flight Demonstration” of 3 April 2009 has been updated to reflect the change in location from China Lake to YTC. The updated instruction is included with this announcement and should be reviewed in its entirety. Offerors are reminded that the Navy will not assume liability for any costs associated with offerors participation in this procurement or demonstration. Within the facilities restrictions or parameters, use of government range time will be provided at no cost to offerors. YPG Visit Request/Visitor Access. Paragraph 5.7 of the “Instructions for the STUAS/Tier II UAS Flight Demonstration” provides information for submittal of official visit requests, including requirements for foreign visitors. Visit requests for personnel participating in the Site Visit are to be submitted no later than 07 May 2009. Visit requests for all personnel participating in the demonstration are to be submitted no later than 21 May 2009. Directions. Directions for YPG visitor access and badges: Drive North on Highway 95 from Yuma Turn Right (east) on Aberdeen Road after mile marker 47 (first right after big guns). The guard post will direct to the Pass & ID section for vehicle passes and visitor ID badges (rental agreement, proof of insurance, identification, and visitor request are required). Directions to Bldg 2887 from Pass & ID section: Drive back to Highway 95 Turn Left (south) on Highway 95 for approx. 75 yards Turn Right (west) on Martinez Lake Road for approx. 1.5 miles. Enter access gate with Huey helicopter display out front. Bldg 2887 is the first bldg on the right with a Cobra Helicopter displayed in front.
