Description
Synopsis: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6. This announcement constitutes the only solicitation and a written solicitation will not be issued. This synopsis, NAICS code 325120, is to notify contractors that the Food and Drug Administration (FDA) is seeking competition of this requirement in accordance with FAR Part 13.106. Under the simplified acquisition procedures and the resultant purchase order will include all applicable provisions and clauses in effect through the Federal Acquisition Circular 05-32. This synopsis is set aside for 100% small business competition for a firm fixed-price purchase order. Prospective contractors are responsible for monitoring the FedBizOpps website for the release of any amendments to this solicitation. Hard copies will be provided to individuals eligible under the Americans with Disabilities Act and Rehabilitation Act upon request. The Government reserves the right to award a contract without discussions if the Contracting Officer determines that the initial offer(s) is/are providing the Best Value and discussions are not necessary. This combined synopsis solicitation is issued as a Request for Quotations (RFQ). Project Title: Carbon Dioxide Solid (Dry Ice) Specifications: Carbon Dioxide Solid (Dry Ice), Pellet Size: 1. Approximately one quarter inch in diameter and varying in length from one eighth of an inch to one and three eights of an inch. 2. Approximately one half inch in diameter and varying in length from one eighth of an inch to two and three quarters of an inch. Deliverables: 1. The Contractor shall arrive each Monday of each week between the hours of 8:00 am and 12:00 pm (noon), Eastern Standard Time (EST) prepared to deliver 2500 pounds of Dry Ice, Rice Pellet size, approximately one quarter inch in diameter and varying in length from one eighth of an inch to one and three eights of an inch, to the Building 29A Loading Dock. The Contractor shall transfer the Dry Ice from the storage bin on their truck to the storage bins that are provided at the Building 29A Loading Dock. 2. The Contractor shall arrive each Monday of each week between the hours of 8:00 am and 12:00 pm (noon), EST prepared to deliver 1100 pounds, Pellet Size approximately one half inch in diameter and varying in length from one eighth of an inch to two and three quarters of an inch, to the NLRC. 3. The delivery truck shall be equipped with a lift gate as there is no loading dock at the Nicholson Lane Facility. The Contractor shall exchange two of their empty 500 pound Dry Ice insulated containers for two of their full 500 pound containers. If only one container is empty, only one container shall be shall be exchanged and the other container brought up to the full mark. The total poundage of the dry ice to be shipped is approximately 130,000 Pounds Rice pellet size to Building 29A and approximately 57,200 pounds Larger pellet size to NLRC for a total of approximately 187,200 pounds. 4. In the event a holiday and/or other Federal closings fall on Monday the delivery shall be made on the next available workday of that week. 5. The Contractor shall adhere to all Local City, County, State, Federal, DOT, EPA, OSHA and NFPA rules and regulations and standards set forth by these organizations. MSDS Sheets shall be provided upon request. 6. The Contractor shall provide Company Letter Head delivery invoices at the time of the delivery, indicating the amount of Dry Ice that was shipped and the amount that was actually delivered. The delivery tickets shall be given to the Project Officer, Jim Rice, or a Designated FDA Representative. 7. The Contractor shall adhere to the safety procedures, rules and regulations in the handling and delivery of the Dry Ice. Spillage of Dry Ice Pellets shall be swept up immediately and discarded in a safe place as to avoid slips and falls by any persons walking in the vicinity of the delivery. 8. Contractor shall send the actual invoice stating the quantity that was actually delivered, to Catrina Posley at Catrina.Posley@fda.hhs.gov to approve for payment so FDA will only be paying for the amount of Dry Ice that was delivered and not paying for the amount that was shipped. The weight recorded on the invoice at the time the Dry Ice was loaded at the Depot will be sufficient. The Food and Drug Administration shall only pay for the amount of Dry Ice that is actually delivered and not pay for the amount shipped. Period of Performance The purchase order award will be for a base year and four (4) option years. Place of Performance Delivery for the FDA, CBER for two separate locations; 8800 Rockville Pike Building 29A Loading Dock, Lincoln Drive Bethesda, MD 20892 Nicholson Lane Research Facility 5516 Nicholson Lane Kensington MD 20895. CCR: Vendors must be registered in the Central Contractor Register (CCR) prior to the award of a contract. You may register by going to www.ccr.gov. You will need your Dun & Bradstreet number and banking information. All responsible sources that can provide and meet the above requirements, shall submit written quotation by the response date. QUESTIONS DUE: All questions must be received by email to: karen.petty@fda.hhs.gov, no later than 5:00pm, EST on or before Wednesday, May 13, 2009. QUOTATIONS DUE: All quotations must be received by email to: karen.petty@fda.hhs.gov, no later than 5:00pm, EST on or before Friday, May 22, 2009. Telephone calls will not be accepted. EVALUATION AND AWARD: Award will be made to the party whose quote offers the best value to the Government, technical, price, and other factors considered. The Government may award this purchase order to other than the lowest price technically acceptable quote. The Government will evaluate information based on the following evaluation criteria: 1).Technical Capability factor "Meeting or Exceeding the Requirement”. The Contractor shall be solely responsible for the quality of their work and the purity of the Dry Ice. The Dry Ice shall be of Standard Commercial Quality. Contractor shall provide a Certificate of Conformance which details the purity level of the Standard Commercial Quality Dry Ice. Those who do not submit a (CoC certificate will not be considered.) 2).Price. Include the base price with four (4) option years. Technical Capability is significantly more important than price, and 3).Past Performance. PROVISIONS and CLAUSES: The provision at FAR 52.212-1, Instructions to Contractors Commercial Items applies to this solicitation. The following agenda has been attached to this provision: None.
