- Dental Requirements The Defense Supply Center Philadelphia, Pacific Region (DSCPP) is requesting urgent quotes for dental supplies listed on the attached Excel spreadsheet. Please provide your unit price, extended amount, quantity on hand on this spreadsheet. This requirement is urgent and the required delivery date (RDD) is on or before 1 Jun 2009, FOB Point: Destination to San Diego, California. The government will make award in the preference/sequence as follows: 1) All-or-None basis to the lowest evaluated price who meets the RDD. If this is not met then 2) Progressive Awards (If the offeror with the lowest evaluated unit price for a line item offers less than the total quantity required, award up to the quantity offered, follow the same procedure with the next lowest evaluated unit price and continue until the entire line item requirement is awarded. If these items are covered under a GSA/VA Federal Supply Schedule (FSS), please provide the FSS contract number and expiration date. (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. (ii) The solicitation is issued as a request for quotation (RFQ). (iii) The purchase request number is 2130521709. This solicitation document incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-30 and DFARS Change Notice (DCN) 20090115. (iv) This solicitation is unrestricted. The NAICS code for this requirement is 339114, Dental Equipment and Supplies Manufacturing. (v) The requirement: See attached Excel spreadsheet. (viii) The provision at FAR 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) Include a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items; Additionally, the following DFARS provisions apply: DFARS 252.212-7000 Offeror Representations and Certifications--Commercial Items; DFARS 252.209-7001, Disclosure of Ownership or Control by the Government of a Terrorist Country; and DFARS 252.225-7000.
