- solicitation revision 2 Amendment 4: This amendment hereby makes the following changes to the Performance Based Statement of Work and solicitation: The paragraph titled "lead waste" in section 3.4.4 was deleted as that applied to the indoor range. Slight changes were made to section 3.1.1 for intake and exhaust chambers Under the "addendum for instructions to offerors" in the solicitation roman numeral "VI" was revised to reference clause 52.232-28 instead of 52.232-38 and roman numeral "VIII" was added under this section to remind you to submit past performance references for subcontractors as well if it was not done in Phase I. All other terms and conditions remain unchanged. *************************************************************** Amendment 2: This amendment is for clarification purposes to clarify Part C of Phase I for the Background Check and Phase II for the site visit. In phase I for the background check, the SF 3237 shall be filled out and submitted for all personnel that will be present at the site visit. The government requests that no more than five personnel from each offeror be present for the site visit. In addition, the offeror that is awarded this contract shall be required to fill out the SF 3237 for all personnel that will be working on the government facility. If any person fails the inital background check, that person will not allowed to access the government facility. If any person fails the inital background check for the site visit, that offeror will be excluded from the competitive range. ****************************** The Government intends to award a competitive commercial fixed priced / time and materials type contract for lead abatement services located in the Washington, DC area. Contract award is intended to be made by 1 July 2009. Award will be made on best value. Evaluations will occur in three phases. The resulting contract will be for one year from 1 July 2009 to 30 June 2010. This contract includes an award term incentive of six one year award terms. If all award terms are earned, the contract shall not exceed seven years. This solicitation is set aside for 8(a). The North American Industry Classification System (NAICS) is 562998. Potential offerors are hereby notified that the solicitation and subsequent amendments to the solicitation will only be available by downloading the documents at the Federal Business Opportunities Website at http://www.fbo.gov. This office will not issue hard copy solicitations. By submitting a proposal, the vendor will be self-certifying that neither the requestor nor the principal corporate officials and owners are currently suspended, debarred, or otherwise ineligible to receive contracts from any Federal Agency. The solicitation, HSSS01-09-R-0025, will be posted on 31 March 2009 by 5:00 PM EST. The deadline for Phase I will be 13 April 2009 by 0900 EST. This notice will be amended to include deadlines for Phase II and Phase III for proposals once the solicitation is issued.
