Description
Item Description Description(s): This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ). Quotes will be evaluated IAW FAR Part 13 best value procedures. This procurement is for the purchase of an all inclusive system for Electrocardiograms (ECG) and Picture Archive and Communication System (PACS). The attached item description identifies the requirement. The solicitation document and incorporate provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-32 (effective 31 March 2009) and Defense Federal Acquisition Regulation Supplement change notice 20081020. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. FAR 52.212-2, Evaluation - Commercial Items also apply. Fill-ins for FAR 52.212-2: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical capability will be evaluated by how well the proposed product meets the government requirement instead of predetermined sub-factors. Evaluation Basis for Award: Technical, past performance and price in this order of relative importance. It is the offeror's responsibility to provide points of contact for at least two (2) current or recent contracts for the same or similar requirements to whom past performance questionnaires may be sent. Past Performance will be evaluated using the following: High Confidence: Based on the offeror's performance record, the government has high confidence the offeror will successfully perform the required effort. Significant Confidence: Based on the offeror's performance record, the government has significant confidence the offeror will successfully perform the required effort. Satisfactory Confidence: Based on the offeror's performance record, the government has confidence the offeror will successfully perform the required effort. Unknown Confidence: No performance record is identifiable. Little Confidence: Based on the offeror's performance record, substantial doubt exists that the offeror will successfully perform the required effort. No Confidence: Based on the offeror's performance record, extreme doubt exists that the offeror will successfully perform the required effort; (b) The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic price. 52.212-3 Offeror Representations and Certifications -- Commercial Items, 52.212-4 Contract Terms and Conditions -- Commercial Items and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, and applicable DoD and Air Force supplement clauses also apply. Wide Area Work Flow (WAWF) invoicing procedures also apply. The type of contract anticipated is firm-fixed price with a one year base period and three option years for maintenance only. Contract Line Item Number (CLIN) 0001 will be for Electrocardiograms (ECG) and Picture Archive and Communication System (PACS) with one year maintenance (see paragraphs 1.1 to 1.4 of the item description) and option CLINs 1001, 2001, and 3001 will be for continuing maintenance of the system (see 1.5 of the item description). The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).The Government's cost target is less than $300,000.00 for this project. It is the offeror's responsibility to be familiar with applicable clauses and provisions. The delivery and installation completion date for these items is four (4) weeks after receipt of Order and if that is not possible then propose a delivery and installation completion date. Delivery will be FOB Destination to Brooks City-Base, TX. The NAICS Code is 6215 the size standard is $13.5M.
