Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81K00-09-T-0188 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32. This acquisition is unrestricted under NAICS code 339112, Size STD 500. The requirements in this solicitation: 1. Harmonic Generator GENO4 Qty 2 2. Cart for Generator 300 (1/Case) CRT01 QTY 2 3. Foot Switch & Cable (1/Box) FSW01 QTY 2 4. Gray Handpiece (1/Box) includes (1) HST02 & (1) TLB01 QTY - 4 5. Harmonic Blue Handpiece (1/Box) QTY 2 6. HS Synergy Curved Blade (6/Box) QTY 2 7. Trade in Credit Qty 1 MFG: Harmonic Distributor: Ethicon Endo-Surgery Vendors offering other than the brand name product shall meet or exceed the following Salient Charteristics: Ethicon Harmonic Generators Model 300, Gen 04 plus accessories that converts electrical energy into menchanical energy (ultrasonic vibration). The vibration is then transmitted from the transducer to the attached instrument for cutting and coagulation. The use of the harmonic generator greatly reduces the damage to patient tissue during surgery. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.211-6 (and other FAR clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, 30 days after receipt of order, shipping shall be FOB destination to Reynolds Army Community Hospital, FORT SILL, OK 73503. Acceptance Location: Reynolds Army Community Hospital, BLDG 4300 Thomas Road, Fort Sill, OK 73503.
