- RFQ Details Custom Foot Orthoses are required for NH Jacksonville, FL for a base year and two one year options. The Government estimates that a quantity of 600 pair are needed each year. The base year period of performance is from 1 OCT 2009 - 30 SEP 2010. Interested vendors should submit all required materials needed to evaluate quote to include pricing, product descriptions, brochures, forms, and description of services.
