- This is a Request for Information (not a pre-solicitation notice pursuant to FAR Part 5) to receive industry comments on the attached Performance Work Statement for performance of non-personal services to conduct patient surveys. This notice is for market research purposes and requests interested sources to provide their companys comments ONLY. The Center for Health Care Contracting, Fort Sam Houston, Texas, seeks interested sources with the required capabilities to provide patient survey services. This initiative is for the Office of The Surgeon General (OTSG), Army, which operates Military Treatment Facilities (MTFs) in both the continental United States (CONUS) and Overseas locations (OCONUS). The types of surveys to be used are Provider Level Patient Satisfaction Survey, Warrior in Transition Unit (WTU) Survey, Benchmark Survey and Provider Survey. The offeror would be required to provide qualified personnel, services, materials, equipment, supplies and facilities necessary to perform required services. This is a Request for Information (RFI) issued pursuant to FAR Part 10. This RFI requests that potential offerors review the attached Performance Work Statement and then submit their comments, questions and industry standards for this requirement to the individual listed below. This notice is for informational purposes only and does not constitute an Invitation for Bid nor Request for Proposal, and is not to be construed as a commitment by the Government. The Government will not reimburse contractors for any cost incurred for their participation in this survey. It is anticipated that any resultant contract that might result from this RFI would be Firm Fixed priced. Period of performance would be a base plus four 1 year options. The North American Industrial Classification System (NAICS) code is 541910; and the small business size standard is $7.0 MIL. Responses are due by 5 June 09 and should contain Company Name, Address, Point of Contact, Cage Code, Phone Number, FAX Number, and comments, questions and industry standards regarding the PWS. Comments, questions and industry standards should include the specific paragraph number being referred to by the responder. Large and small businesses are encouraged to participate in this RFI. Interested sources should submit their responses and any questions regarding this requirement in writing to Liza Esmond, Contract Specialist, at Liza.Esmond@amedd.army.mil or Johnie Johnson, Contracting Officer, Johnie.Johnson@amedd.army.mil prior to the close of this notice. Responses are due by 5 Jun 09.
