Description
On or about 16 June 2009, this office will issue Request for Proposals for construction of the Elbowoods Memorial Health Care Facility, Fort Berthold Reservation, New Town, North Dakota. The solicitation will close on or about 23 July 2009. This solicitation is RESTRICTED TO SMALL BUSINESS PARTICIPATION. Contractors interested in inspecting the site of the proposed work should attend the scheduled site visit. The site visit is planned for June 30, 2009 at 9:00 am at the project site. Reservations need to be made with Loren Nishek, U.S. Army Corps of Engineers, at 701-727-6127. DO NOT submit requests for plans and specifications to the site visit personnel listed above. See Obtaining Solicitation Documents below. _______________________________________________________________________________ Project Description: (Approx. quantities) The scope of the project includes all work required to construct the Elbowoods Memorial Health Care Facility in New Town, North Dakota for the Three Affiliated Tribes (TAT). The construction will include a primary health care facility, EMS, and supporting site work infrastructure (utilities, sidewalks, drainage, landscaping, parking, etc.) necessary to provide a complete and usable turnkey facility. Road infrastructure will also be designed and constructed to the limits designated. The facility shall have a minimum of fifteen (15) exam rooms. The facility will also include Environmental Health, Health Education, Public Health Nursing, and Public Health Nutrition. The scope also includes all loose furniture such as chairs, tables, desks, waiting room furnishings, and specialized medical equipment needed to support clinic operations. The estimated construction cost of this project is between$15,000,000 and $20,000,000. _______________________________________________________________________________ The Contractor will be required to commence work within 10 days after notice to proceed and complete the work 540 calendar days after receipt of Notice to Proceed. Provisions will be included for liquidated damages in case of failure to complete the work in the time allowed. Performance and payment bonds will be required. Bonding documents including the power of attorney shall bear an original signature by an officer of the surety. Representations and certifications are required to be updated annually as a minimum to keep information current, accurate and complete. The electronic annual representations and certifications are effective until one year from date of submission or update to ORCA.
