- This is a Combined Sysnopis Announcment and Solicitaion for Dental Exams.Treatment and Periodic Health Assessment. Contractors need to know that this is the only announcment and document submitted in order to seeks quotes for this requirement. NAICS 621399 will be considered as well for this procurement Due to the Short Suspesnse this requirment will be procured on a Full and Open Competition Basis. The Arkansas Army National Guard has a requierment for Mobile Dentist Treatment/Exams and Periodic Health Assessment to be conducted at Fort Chaffee Arkansas during the p;eriod 24-25 June 2009 for Approximatley 400 Soldiers SERVICES requested: Dental exams and Dental treatment DENTAL EXAMS will be include the following: Bitewings Panoramic radiography, if required All x-rays will be digital and loaded into DENCLAS within 5 days after the event All dental exams will be loaded into DENCLAS and the results from the exam will be annotated in the dental record Class 3 dental exam's will be indentified and prepped for same day dental treatment Dental exams will be conducted with dental explorer and mouth mirrors. The Periodic Screening Assessment (PSA) will be annotated as a part of the dental exam also Dental exam classification will be annotated by the following: Dental Class 1-Patient has good health and is not expected to require dental treatment or reevaluation for 12 months. Dental Class 2-Patient has some oral conditions, but you do not expect these conditions to result in dental emergencies within 12 months if not treated, (i.e., requires prophylaxis, asymptomatic caries with minimal extension into dentin, edentulous areas not requiring immediate prosthetic treatment). Dental Class 3-Patient has oral conditions that you do expect to result in dental emergencies within 12 months if not treated. Dental Class 4-Patient requires a current (within 365 days) dental examination in or to determine if they are Class 1, 2 or 3. A missing panoramic radiograph is also a dental class 4 condition. DENTAL TREATMENT: All Soldier's identified as Dental Class 3 will receive the appropriate dental treatment plan and consultation required to successfully transition Soldier from Dental Class 3 to Dental Class 2, (See attached enclosures Dental Classification). Contractor will provide and prescribe to Soldier's identified as Dental Class 3 requiring dental treatment, the appropriate prescription or medication as a part of treatment Contractor will track, monitor, and follow up with any dental concerns or issues identified by Soldier's who were treated Periodic Health Assessments (PHA) Contractor will conduct basic Periodic Health Assessments to include the following at buildings 1339 and 1340 Vision screening, Immunizations as identified in the Medical Protection System (MEDPROS): Hepatitis A or B series; Tetanus; Diphtheria; Influenza, others as identified by medical staff Conduct Medical Screenings on all Soldiers to include height and weight, blood pressure check, ECG (Electro Cardiogram) Hearing screening utilizing the required equipment Lab, only if needed and as identified in the Medical Protection System (MEDPROS), HIV, DNA, CVSP (over 40 blood draw), G6PD, PAP smear screening (female only) Contractor will treat Soldier with respect and dignity Contractor will provide all required resources to accomplish this event, (i.e., mobile equipment, vehicles, personnel, medication, etc). Contractor's personnel will have all medical certifications visually displayed for Government review. Contractor will ensure that the facilities utilized during this event is cleaned and restored back to its original state. Contractor's mobile unit/s will be equipped and OSHA compliant and all other certifications and compliant rules associated with the unit/s and equipment. Compliant certifications will be inspected by the Government. Contractor will ensure that all background checks are performed on all employess and these background checks and names of all personnel be presented to Fort Chaffee Department of Public Safety upon commencement of contract. Hours of operation for this requirement will begin at 07:00am on the 24th. In order to complete this requirment by the end Close of Business on the 25th of June, contractor and Contracting Officer Representative (COR) will coordinate hours of operation.
