- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 and Far Part 13 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is being issued as Request for Quote #VA-246-09-RQ-0286. The solicitation document and incorporated provisions and clauses are those in effect through the most current Federal Acquisition Circular. This requirement is 100% set-aside to Service Disabled Veteran Owned small business under NAICS Code 423450. The Department of Veterans Affairs Medical Center, Salisbury, N.C. has a requirement for a narrow band imaging ureteroscope and the appropriate instrument tray for transport and storage of the instrument. This is the request for quotes and will award a purchase order based on quotes received. The ureteroscope must have narrow band imaging capabilities and be a flexible digital scope. The scope MUST be compatible with the existing Olympus Visera Pro Video processor and light source and Siemens equipment. Item # 1- Narrowband flexible digital imaging ureteroscope - 3 each. Item # 2 - Instrument case - 3 each. The following provisions apply to this solicitation: 52.212-1, Instructions to Offerors-Commercial, 52.212-4, Contract Terms and Conditions-Commercial Process of determining contract award. A completed copy of provisions 52.212-3, Offeror Representations and Certifications, Commercial Items must be submitted with offer. Award will be based on the lowest price, technically acceptable offer. Proof of compatibility with existing equipment is to be presented with the quote. Offerors desiring to obtain copies of the reference provisions may access clauses electronically at http://www.arnet.gov/far/ or http://www.va.gov/oa&mm/vaar. Quotes must be submitted by an authorized company representative. Company name, address, DUNS number, and Federal Tax ID number must be included with the quote. Registration in the Central Contractor Registration (CCR) database is required prior to award of purchase order. Quotes must be received by June 17, 2009, at 12:00p.m. local time. Quotes shall be submitted electronically to barbara.simmons@va.gov. If mailed the address is: Contracting Office, Building 27, Room 109, ATTN: Barbara Simmons, Hampton, VA 23667. Any questions must be presented in writing via e-mail not later than June 12, 2009.
