Description
The Gerald R. Ford Job Corps Center is soliciting cost proposals for qualified professionals to provide subcontracted medical services to students (age 16 - 24) for the following position(s): Optometrist Services are to be provided on a weekly basis for several hours for each class of 3 - 15 new enrollees for 48 weeks per year. Services are to include: pricing for comprehensive exam, intermediate exam, limited exam and glasses. Cursory exam will include recording a history, present complaints, complete retinoscopy, Refraction, Autorefractors and Aberrometers, Cover Test, Slit-Lamp Examination, Glaucoma Test, Visual Field Test and Dilation. Patient's visit will provide a subjective examination of external eye and lids, eye movements and pupil reflexes, writing RX to explain to patient examination results, assist patient in frame selection and proper eyeglass prescription from an approved laboratory, proper adjustment and alignment of the finished glasses to the patient's face. New eyeglasses will include first quality single vision CR-9 or comparable plastic lenses, with prescription up to plus or minus four diopters sphere power and up to minus three diopters of cyclinder power. Higher prescriptions will be provided at a charge of $___ per lens per diopter of sphere and/or cylinder power beyond that indicated. Optometrist should also include price of high index lenses for students who require thick lens. Any other options such tints and scratch coating should be quoted, but will be authorized on a case-by-case basis through the Gerald Ford's Wellness Center Manager. Please provide cost of additional services such as foreign body removal, treatment, or eye injuries or infections, visual field evaluations, etc. Contact lenses should be priced, but will only be provided when authorization is given in writing by Wellness Center Manager. Proposal must provide a copy of current license, proof of malpractice insurance, Resume', rates as outlined in this RFP, and name and credentials of Office Associates who will be treating our students.
