- This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications --Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; and 52.212-5 Contract terms and Conditions to Implement Statutes or Executive Orders - Commercial Items; FAC 05-32-Technical Amendment. NAICS Code is 332311. The proposed procurement is unrestricted. Reference number: N6847009RQ27113. The U.S. Naval Hospital, Okinawa, Japan (NH Okinawa) has a requirement for an audiometrics booth similar to that of the Industrial Acoustics Company’s (IAC) Accutone 2 (p/n: 120a-2). The quoted audiometrics booth must be double-walled (room within a room) with outside dimensions of approximately 8’ x 8’ x 8’ (LxWxH) and inside dimensions of 6’ x 6’. The booth must weigh 5,338 pounds or less. The contractor shall be responsible for shipment of the audiobooth to a point designated by NH Okinawa, which will be either the Government depot located in Tracy, California (zip 95304) or direct to Okinawa, Japan, if possible. A quotation shall be provided for each location. The contractor shall also be responsible for installation at the NH Okinawa, inclusive of all labor, travel and lodging, tools and equipment, and other related travel and installation costs. Supporting evidence and literature must be furnished in sufficient detail that demonstrates the contractor’s capability to provide and ability to comply with the above requirements. The Government will award a contract resulting from this solicitation to the responsible contractor whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors will be used to evaluate offers: (1) Technical capability of the item offered to meet the Government requirement (including submission of descriptive literature); (2) Price; all to be submitted with quotation. No telephone requests will be accepted. Faxed quotes will be accepted. Contractors are responsible for obtaining any and all amendments or additional information. Quotes and all pertinent data (descriptive literature, specifications, etc.) must be submitted to the POC no later than 15 June 2009.
