Description: 1.SPECIAL NOTE: THIS IS TIME SENSITIVE AND DELIVERY AND INSTALLATION SHALL BE WITHIN 30 DAYS OF AWARD RECEIPT. IF THE OFFEROR CANNOT PROVIDE THE SERVICES WITHIN 45 DAYS ITS OFFER WILL NOT BE CONSIDERED. ALL PROPOSALS WILL INDICATE THE MAXIMUM NUMBER OF DAYS IT WILL TAKE TO DELIVER ALL EQUIPMENT. 2.This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 3.Request for Quotation Number W911S1-09-T-0017. 4.The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-18 effective 5 July 2007, DFAR DCN 20070531, and AFFAR AFAC 20070531. FedBizOps Numbered Notes per FAR Part 5.207-13(e) apply: 12 and 13. 5.This procurement is being solicited restricted 100% for small business under North American Industry Classification System code 423990, Monuments and grave markers merchant wholesalers, with a size standard of 100 employees. The Standard Industrial Classification (SIC) is 5099, Monuments and grave markers merchant wholesalers. 6.Contractor shall provide all equipment, personnel, labor, supplies, tools, materials, supervision and other items necessary to replace, upgrade, and install audio visual equipment for the Fort Dix, NJ Chapel (see equipment list). 8.Award will be made to that responsive, responsible offeror whose proposal conforms to all terms and conditions of the solicitation and is most advantageous to the government. 9.Proposals are due not later than 12:00a.m. EST, 19 June 2009. ITEM NOSUPPLIES/SERVICESQUANTITYUNITUNIT PRICEAMOUNT 0001 Markers for Museum Displays FFP The contractor shall provide all labor, equipment, tools, materials, supervision, and transportation necessary to provide (41) bronze plaques mounted at a 45 degree angle on an exposed aggregate base for museum displays at Bldg 6501 Route 608 Circle, Wrightstown Circle, Building 5165, Infantry Park, Visitors Center, Doughhboy Field, Building 5634, and Pointville Entrance. Bronze plaques shall be 18W x 15H, with raised bronze letters, mounting screws shall be covered with oak leaf clusters, provide a six inch (6) reveal around plaque when mounted on exposed aggregate concreate base. Exposed aggregate concrete base shall have a yellow pea gravel finish. Contractor shall deliver and set the plaques in place at various specified locations, identified above, on Fort Dix, New Jersey. Contractor shall replace existing concrete based markers with an exposed aggregate concrete base marker and new bronze plaques at Infantry Park. Contractor shall restore existing bronze plaques set in lare free standing boulders Fort Dix field house to the original look and apply a UV protected top coat.
