- JOFOC file The Centers for Disease Control and Prevention (CDC), National Center for Injury Prevention and Control (NCIPC), Division of Unintentional Injury Prevention (DUIP) intend to award a sole source, two year firm fixed price contract, in accordance FAR Part 12- Acquisition of Commercial Items to The State and Territorial Injury Prevention Directors Association (STIPDA), 2200 Century Parkway, Suite 700, Atlanta, GA 30345. STIPDA has unique and detailed data about the status and capacity of all the state injury programs. STIPDA conducts a biannual survey of all 50 states and publishes the State of the States Report that provides a comprehensive picture of the status of the nation's state health department injury programs. This data will allow STIPDA to identify pilot sites that have the capacity and infrastructure to successfully conduct fall prevention programs. STIPDA is the only entity with access to this data set. CDC anticipates soliciting and negotiating with only one source under the statutory authority 10 U.S.C. 2304(c)(1) permitting other than full and open competition in accordance with Federal Acquisition Regulation (FAR) Part 6.302-1 - Only one responsible source and no other supplies or services will satisfy agency requirements. NCIPC DUIP seeks external expertise to assist in six main areas: 1) Identifying appropriate CORE Injury pilot states. 2) Providing training and technical assistance to state health departments and partners to deliver community-based falls programs including two evidence-based falls prevention programs, Stepping On and Moving for Better Balance, this will be used in selected states. 3) Overseeing program implementation in community settings within the selected states. 4) Developing and testing support materials to enhance implementation. 5) Supporting the collaboration of state falls coalitions, the aging network, and state health departments around fall prevention activities and programs. This two part project will require that the contractor to perform the following: Part A. A Pilot Implementation of Community -Based Fall Prevention Programs through State Health Departments: • Work with DUIP to identify two pilot states; • Set up a mechanism to fund one or two programs in each state. • Develop, organize and deliver a training course to state health department staff in Atlanta, Georgia. This will require developing new training and support materials. • Work with the two identified state health departments to oversee implementation of one or two pre-identified falls prevention programs. • Provide on-going training, monitoring, technical assistance, and coordination to state health department staff who are implementing the program. • Develop a feasibility assessment tool. • Provide a report summarizing the technical assistance needs of the state health departments during this project. • Provide a report summarizing what has been learned from working with the two health departments, including the process of establishing partnerships, developing program logistics, reaching older adults, and delivering the program. Part B. National and State-Level Implementation of the 2006 Field Triage Decision Scheme: • Identify and collaborate with appropriate national EMS organizations to increase implementation of the 2006 Field Triage Decision Scheme. • Develop and implement a nationwide implementation plan. • Develop state-level strategic implementation plans. • Monitor and communicate successes in the adoption and implementation of the 2006 Field Triage Decision Scheme by all levels of EMS systems. Therefore, this contractor must have an excellent understanding of state health departments and injury activities as well as state-of-the art training and technical assistance techniques appropriate for this audience. CDC believes that STIPDA is the only source available to provide this product that meets mission critical requirements. This notice of intent is not a request for proposal. A determination to compete this procurement based on a response to this notice is solely within the discretion of the Government. This procurement is not set-aside for small business. The NAICS Code for this requirement is 813920.
