Description
This is a combined synopsis/solicitation for a lease of a compact, digital nuclear medicine imaging system for a period of six months with optional 30 day extension, prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued through FedBizOpps. The Louis A. Johnson VA Medical Center intends to award a firm fixed price commercial services contract in response to the Request for Quote (RFQ) Number is VA-244-09-RQ-0268. Copies of the complete RFQ are available upon request. The solicitation documents and incorporated provision and clauses are those in effect through Federal Acquisition Circular 2005-30. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The clause may be accessed in full text at this address www.acqnet.gov/far. The NAICS code is 811219. The six months lease will begin at time of delivery and set up. The camera must be able to provide, at a minimum, capability for cardiac SPECT imaging with or without additional planar imaging capability. Proposed price should not expire before September 30, 2009. Award of this six month contract is expected to be made sometime after the close of this RFQ and before September 30, 2009. All leased equipment must be delivered and set up and initial training must take place within 45 days of award of the contract. Lease Agreement Required Components (Included in quoted price): "Delivery and set-up of equipment is the responsibility of the lessor "Warranty on camera is the responsibility of the lessor; all parts, labor, and travel must be included during VA use of the camera "Lessor will provide initial technical support after installation and continue for the remainder of the lease "Lessor will provide at least two days on-site training for the current VA technologists and at least one day on-site training at our affiliate; travel and lodging cost to be borne by the lessor "Camera must include a processing workstation equipped with software appropriate to the capabilities of the camera in use "Additionally, a second reading station must be provided for remote interpretation by our affiliate "Lessor must provide means for archiving raw data on an external media source (i.e. optical disk, CD, DVD, etc.) "Lessor will remove and dispose of all shipping containers "Rigging and/or etc. will be incorporated into the quoted price "Lessor will provide one set of operator's manuals "Lessor must provide a DICOM connection "Any hard drive containing VA patient information must be removed and destroyed by VA staff upon contract end. Any fee for replacing the hard drive must be included in the quoted amount. FSC may utilize third-party contractors to facilitate invoice processing.
