Description
State of idaho Certificate of Verification of Worker's Compensation Insurance. Wage Determination This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quote (RFQ) AG-0281-S-09-0048. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32, effective as of May 14, 2009. This acquisition is 100% set-aside for Small Business Concerns. NAICS is 562991; small business size is $7 M. This project is located at the Coeur d’Alene Interagency Fire Center, 11569 Airport Drive, Hayden, ID. The period of performance of this contract is from 7/1 through 10/15/2009. Fire season activity at the Dispatch Center dictates these dates. The effective base period of this contract is from 7/1/2009 through 9/30/2009, with 4 option years. Description of Commodity: This work involves pumping and removing pond effluent as needed, for the 2009 fire season, when the pond reaches 80% capacity. The pond size is 30 feet X 50 feet. The contractor is responsible for disposing of the effluent in the same manner as septic waste. The contractor is responsible for compliance with any state or local regulations. Material Safety Data Sheets on retardant and a lab report of the pond contents is available. A minimum 5,000 gallon truck tank capacity is required. For safety reasons during the busy fire activity, a larger truck is desired to limit the number of trips to the pond which requires a government escort. The government estimates there will be a need for approximately 6 to 8 pumping(s) throughout the fire season. The Contractor agrees to perform this work on an on-call when needed basis by the Government. Twenty-four hour notice can be given. The amount of fire activity will dictate the frequency of pumping(s) required. Price per job or gallon, for pumping and disposal including all equipment, labor, materials, travel and permits. List all contracts for similar work, along with name and phone number, within the last three years. 2) Specify year, make and model, and gallon capacity of equipment to be used. 3) Provide an explanation of how and where effluent will be disposed of. Tax ID number_______________ and Duns number_________________________. Offeror must be registered in ccr.gov. The following provisions and clauses are applicable to this acquisition and may be viewed at http://arnet.gov/far.html. The provisions at 52.212-1 Instructions to Offerors -- Commercial Items; is applicable and is tailored as follows: Paragraph (h) is deleted and FAR 52.212-2 - Evaluation Commercial Items; in addition to price and price-related factors, award evaluation will be based on past performance, equipment, and compliance with solicitation requirements. These three evaluation factors evaluated together are equal to price. In addition to price, offeror should include information addressing the above three factors in their offer; delete clause (c). FAR 52.212-3 - Offeror Representations and Certifications - Commercial Items applies to this solicitation. The contractor shall either complete the on-line Offeror Representations and Certifications OR RETURN A COMPLETED COPY OF THE Offeror Representations and Certifications with their quotation. The Online Reps and Certs Application Representations and Certifications Application (ORCA) can be found at http://www.bpn.gov or a hard copy of the provision may be attained from http://arnet.gov/far.html. Clauses 52.212-4 - Contract Terms and Conditions - Commercial Items and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items apply to this acquisition. The following FAR clauses identified in FAR 52.212-5 are considered checked and are applicable to this acquisition; 52.219-6 - Notice of Total Small Business Set-Aside; 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.232-36, 52.232-33, 52.232-36. The following clauses are added: FAR 52.217-9 - Option to Extend the Term of the Contract (fill-in information: Paragraph (a) 30 calendar days in both places, Paragraph (c) 60 calendar months. Attached is Wage Determination #2005-2159, Rev. 7, dtd 5/26/2009 and STATE OF IDAHO CERTIFICATE OF VERIFICATION OF WORKERS' COMPENSATION INSURANCE which is a part of this solicitation.
