Description
1) Type of Notification: Sources Sought This is a sources sought synopsis only. The purpose of this notice is to gain knowledge of potential qualified sources. Responses to this notice shall include the following: (a) company name; (b) address; (c) point of contact; (d) "number of employees" as defined by the Federal Acquisition Regulation (FAR) part 19.101; and (e) the amount of "annual receipts" as defined by the Federal Acquisition Regulation (FAR) part 19. In addition, the respondent must provide a capability statement that addresses the organization's qualifications and ability to perform as a prime contractor the work described below. 2) Description: Personal Services (Clinical Optometrist - 1 FTE) The following is a brief summary of the primary duties that the Clinical Optometrist will perform. This list is not all inclusive: Shall provide routine and specialized optometric care, including the diagnosis, management, and treatment of diseases and injuries of the eyes. Performs examinations and special testing for basic trainees. Precepts student externs assigned to the Optometry Flight. Provides didactic and clinical direction to residents assigned to the Air Force Optometry Residency Program. Performs routine and follow-up care for active duty members enrolled in the Air Force Aircrew Soft Contact Lens Program. Performs initial and follow-up care for patients enrolled in the Aviation/Special Duty and Warfighter Corneal Refractive Surgery Programs. 3) Qualifications: The Optometrist must have a Doctor of Optometry degree from an accredited optometry college in the United States or Puerto Rico. 4) Experience: The Optometrist shall have at least 5 years of uninterrupted clinical experience. 5) Place of Performance: San Antonio Texas Department of Defense Military Treatment Facilities All prospective offerors who may be interested in participating in a future procurement for this requirement should respond to this Source Sought Notice by contacting: Tara Sidwell Address: 37 CONS, 1655 Selfridge Ave., Building 5450 Lackland AFB, TX 78236-5253 E-mail: tara.sidwell@lackland.af.mil Phone numbers: (210) 671-6017
