The following information will be needed to complete forms that will be returned with your proposal: HCR # 430P3 & 430P5 Solicitation # 150-76-09 & 150-82-09 Date of Solicitation – 06/10/2009 Begin Contract Date – 08/15/2009 End Contract Date - 06/30/2013 For Mail Service Between: City, St – Columbus P&DC, OH City, St – Columbus Metro Area, OH Required Vehicles (9) Straight Trucks w/lift gate (2) 4 x 2 Off Road Yard Tractors (9) Two Axle Single Driver Tractors Attachment labeled "Phase One Package” must be submitted NLT June 29, 2009. Completed forms will need to be sent to the following address: EASTERN AREA OFFICE SOLICITATION NO. 150-76-09 & 150-82-09 5315 CAMPBELLS RUN RD PITTSBURGH PA 15277-7070 Supplier must make sure to include the Solicitation number in the address. All documentation for proposal must be received by the above address no later than June 29, 2009 by 1500 p.m. Provide a photocopy of your Social Security Card or IRS Form CP 575 (EIN) *REQUIRED* NO SOLICITATION PACKAGES WILL BE SENT FROM THE CONTRACTING FACILITY OR THE AREA OFFICE.
