- 1) Type of Notification: Sources Sought This is a Sources Sought synopsis ONLY. The purpose of this notice is to gain knowledge of potential qualified sources. Responses to this notice shall include the following: (a) Company Name; (b) Address; (c) Point of contact; (d) "Number of Employees" as defined by the Federal Acquisition Regulation (FAR) part 19.101; (e) The amount of annual receipts" as defined by the FAR part 19 and (f) Cage Number. In addition, the respondent must provide a capability statement that addresses the organization's qualifications and ability to perform as a prime contractor for work described below. 2) Description: Person Services ( Pediatric Nurse Practitioner) The following is a brief summary of the primary duties that the Pediatric Nurse Practitioner will perform. This list is not all inclusive: Provide Nurse Practitioner services such as supervise, provide general screening, medical care, examinations of patients for routine, acute and chronic conditions involving any and all organ systems, provide immunizations, diagnose, treat, and counsel patients. Collaborate with the physician in the health care of patients with chronic illlnesses. Ensure compliance with standards of care and practice in accordance with all established policies, procedures and guidelines used in the Medical Treatment Facility (MTF) and provide care within ethical and legal boundries. Serve and participate in committes, functions and other meetings as directed. 3) Qualifications: Education: Health Care Provider (HCP) shall be a graduate of an accredited Pediatric Nurse Practitioner program, with a Master's Degree in Nuring or equivalent and Board Certified as an Pediatric Nurse Practitioner. The HCP must possess and Maintain an active current, valid, and unrestricted license from a US jurisdiction before practicing independently within the defined scope of practice for their specialty. The HCP shall have and maintain a current Basic Life Support (BLS) certification and Advanced Cardiac Life Support (ACLS) certification. Experience: The HCP shall have provided Pediatric Nurse Practitioner service for a minimum 11 months within the past 12 months. The provider shall also have at least 3 years of experience as a Pediatric Nurse Practitioner. 4) Place of performace: Locations include, but are not linmited to: Wilford Hall Medical Center (WHMC) at Lackland AFB; and/or other military installations in San Antonio TX.
