Description
The Department of Veterans Affairs (VA) Health Care System, 4500 S. Lancaster Road, Dallas, Texas 75216, is seeking sources to provide the outpatient colonoscopy procedures, as described in the attached Statement of Work. Parties capable of providing these services must respond in writing to the attention of the Contracting Officer listed in this announcement byJune 19, 2009, at 2pm local time. Capable parties must also provide in writing, their business size and description/type. This is the only notice of this procurement and no written solicitation will be available. 1.0 SCOPE Contractor shall provide colonoscopy services to eligible beneficiaries of the Department of Veterans Affairs as referred by VA North Texas Healthcare System (VANTHCS) to include various counties throughout North Dallas, and Tyler, Texas. All services are to be performed at the contractor's facility. 2.0 STATEMENT OF WORK 1.The VA will provide a list of eligible beneficiaries to the contractor. Contractor shall schedule appointments directly with eligible beneficiaries. 2.The contractor will communicate with VANTHCS GI staff to ensure seamless continuity of care for the patients. Patients will be contacted no later than 30 calendar days of receipt of paperwork to schedule appointments. 3.The contractor will track and document all cases in which a colonoscopy is incomplete; missed appointment(s); and declination of services by patient(s). These instances shall be communicated promptly, in writing within 5 calendar days of incident, to the Director of the GI Endoscopy Unit. 4.The point of contacts at VANTHCS for VA patient referrals is the Director of GI Endoscopy Unit and Nurse Manager. 5.Travel to the contractor's location will be the responsibility of the veteran. 6.Within ten (10) days of a procedure, the contractor shall send a copy of the procedure reports and pathology reports to the VANTHCS GI Unit, 4500 S. Lancaster Rd, Dallas, TX 75216 for incorporation into the patient's Computerized Patient Record System (CPRS). 7.The contractor will process specimens and must provide a paper report signed by the provider. 8.The contractor shall instruct patients of pre and post procedure care, and provide all bowel preps. Patients shall be instructed to contact Dallas VA Primary Care Provider for all post procedural follow-up. For complication occurring within the first 48 hours to ten (10) days after the procedure, patients must contact VA Nurse Triage Line at (800) 849-3597 EXT 72081 or the nearest medical facility offering emergency services. 9.VANTHCS Dallas will ensure procedure follow-up and care once the procedure report, signed by performing physician, and pathology results are made available to the Director of the GI Endoscopy Unit (Dr. William Harford). All follow up care will be done at Dallas VA. Contractor shall not advise patient to come back to their facility for follow-up. 10.Abnormal results and/or pathology should be reported immediately to Director GI Endoscopy Unit (Dr. William Harford) at 214-707-5910. If unable to contact him, please call 214-857-1590 to speak with GI attending physician. 11.In the event of a life-threatening emergency, the contractor shall have patient transported to nearest medical facility for treatment and contact the VA Administrator on Duty (AOD) at (214) 857-1340 once patient is stabilized. The AOD will initiate a report of contract that will be forwarded to the Medical Service and Fee Basis Staff. 12.VA patients will receive the same consideration for scheduling on a first-come-first serve basis as any other patient, except in a medical emergency. Contractor will comply with all statutes and regulations applicable to special patient populations such as blind patients, spinal cord injury patients or other patients covered under the Americans with Disabilities Act. A determination by the Government to compete the proposed procurement based on the responses to this notice is solely within the discretion of the Government. This notice does not constitute a solicitation or a commitment by the Government to later publish a solicitation. No solicitation document is available. Capable parties must submit their ability to provide these items in writing to the attention of the Contracting Officer listed in this announcement by 2pm local time on June 18, 2009.
