- This is a SOURCES SOUGHT NOTICE. This is NOT solicitation for proposals or quotes. The purpose of this Notice is to obtain information regarding: (1) the availability and capability of qualified sources that can provide Acute and/or Chronic Ventilator Rehabilitation Services at MULTIPLE SITES in MASSACHUSETTS. Interested businesses should indicate their status as: Service Disabled Veteran Owned Small Business, Veteran Owned Small Businesses, HUBZone small business, 8(a), Small businesses with other socio-economic status or Small businesses with no special socioeconomic categories or large business; and (2) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. The NAICS code for this service is 623110 and the size standard is $31.5 million. (3) Their locations within Massachusetts where services can be provided. THIS IS STRICTLY MARKET RESEARCH THAT WILL ASSIST THE DEPARTMENT OF VETERANS AFFAIRS, BOSTON HEALTHCARE SYSTEM PLAN THEIR ACQUISITION STRATEGY. Proposals will not be accepted at this time.General Statement of Need: The information provided is NOT intended to be a solicitation for proposals but rather for potential small businesses in determining their capability to meet requirements. The contractor shall provide acute/chronic ventilator rehabilitation care as specifically authorized by the Department of Veterans Affairs. The care provided shall include: physician, nursing, pulmonary, respiratory and other therapies (i.e. OT, PT,) and pharmacy. Patients shall be transported from the VA Healthcare System, West Roxbury Campus, Brockton Campus or other aras to the contractors facility at no expense to the contractor. Patients on acute ventilator status shall be reviewed 3 months from admissions to the contractors facility, and each additional month thereafter while determine to be on acute ventilator status. The contractors facility shall be responsible to document the review of the acute ventilator patient on the VA Boston Patient Status Review Form. Patients on chronic ventilator status are required to be reviewed every 90 days from the date of their chronic ventilator status determination. The contractors facility shall be responsible to document the review of the chronic ventilator patient on the VA Boston Patient Status Review Form. Definitions: a. Acute Ventilation Rehabilitation: Patient must be ventilator dependent requiring mechanical ventilation for part of the day, as tolerated by the patient, despite attempts at weaning at the referring hospital. If there have not been attempts to wean the patient from the ventilator, it must be documented why weaning was not attempted; and Patient must required multiple and/or complex interventions by specialized staff (e.g. parenteral (additional) charge above daily rate), enteral services, wound care, medication adjustment, I.V., antibiotics, respiratory care, Physical/Occupational Therapy). The patient must have the potential for improvement to the degree that less respiratory/medical support or professional intervention will be required for ongoing maintenance, and/or the patient can be released to a lesser level of care. b. Chronic Vent Rehabilitation: A know vent dependent patient who would be admitted to the hospital for other rehabilitation or complex medical care who has a known discharge plan. The care must include a relatively intense rehabilitation program (i.e. three hours of combination of therapeutic services including physical and occupationaltherapy), as tolerated by the patient, with a realistic plan for discharge in a reasonable period of time or; If the patient is to ill to participate in the above therapy at the time of admission then the patient must require multiple and/or complex interventions by specialized professional staff (e.g. parenteral (additional) charge above daily rate) enteral services, wound care, medication adjustment, IV antibiotics, respiratory care etc.and have a realistic discharge plan. c. Acute Vent Weaning with Vented Dialysis Patient: The definition for Acute Vent Rehabilitation plus the patient who is receiving dialysis treatment 2-3 times per week. d. Off Tour Hours Monday through Friday from 4:30 PM to 8:00 AM, Saturday, Sunday and Holidays. Per Diem Rates: Unless specifically excluded in this contract, the per diem rate established will include the services listed above and will also include all services or supplies normally provided other patients by the facility without extra charge. Services excluded are TPN and hemodialysis. Absences of the patient from the hospital/rehabilitation center in excess of 48 hours will not be reimbursable except with prior approval of the VA Medical Center. The hospital shall not maintain nor provide dual or segregated patient facilities that are segregated on the basis of race, creed, color or national origin. The hospital shall neither require such segregated use by written or oral policies nor tolerate such use by local custom. The term facilities shall include but not be limited to rooms, wards, sections, drinking foundations, and entrances, and other like areas. It is agreed that VA will have the right to inspection of the contractors facility and all appurtenances by authorized VA representative(s) to ensure that acceptable standards are maintained and that the necessary care to maintain the well being of the patient is rendered. It is agreed that duly authorized representatives of VA may provide follow-up supervision visits to the patient to assure the continuity of care. It is understood that these visits do not substitute for nor relieve the contractor in any way of the responsibility for the daily care and medical treatment of the veteran. The contractor shall communicate routinely (at least weekly for acute rehab patients) with the VA Case Manager to discuss the patients status and success in weaning or becoming more independent, requiring a lesser level of care. All medical regards concerning the veterans care in the contractors facility shall be readily accessible to the VA. Upon discharge or death of the patient, the contractor shall retain medical records for a period of at least three years following termination of care. Patient records will be maintained in conformance with the Privacy Act of 1974 (5 U.S.C. 552a). The veteran shall be transferred to the contractors facility at the expense of the VA for a period not to exceed thirty (60) days, unless specifically authorized (with prior approval) by the VA physician managing the case through the VA case manager. It is agreed that the contractor shall submit a report on the medical status of the veteran receiving care to the VA along with the invoice for the care under this contract. The contractor shall coordinate with the VA Case Manager, PMR&S and/or VA Case Manager Geriatrics and Extended Care for the transition of the patient from the acute or chronic respiratory rehab level of care to a different, most appropriate level of care. This is NOT a Request for Proposals and does NOT constitute any commitment by the Government. Responses to this Sources Sought Notice shall be used by the Government to make appropriate acquisition decisions. All interested sources must respond to future solicitation announcements separately from responses to this market research/Sources Sought Notice. Responding businesses should have an active CCR registration, DUNS number and up to date ORCA registrations. SDVOSBs and VOSBs MUST be registered in VetBiz.gov
