Description
The Veterans Integrated Service Network #04 (VISN04) Logistics Office (529 / Bldg. #19 / Rm. 110), located at 325 New Castle Road, Butler, PA 16001-2480 is soliciting for a Community-Based Outpatient Clinic (CBOC) for providing Primary Care Services. The contractor shall provide the services of a CBOC that must be physically located within Clarion County in Pennsylvania. The Contractor shall provide Primary Care Services in a private hospital, office or clinic environment to veterans primarily residing in Clarion County. The solicitation #VA-244-09-RP-0173 has a NAICS code of 621498 and a Size Standard $10.0 million dollars with a Firm Fixed Price contract being anticipated. Primary Care services will provide a continuum of care from prevention to diagnosis and treatment, to appropriate referral and follow-up. Those patients needing specialty or follow-up care shall be referred to VAMC Butler. A CBOC must have the necessary professional medical staff, diagnostic testing and treatment capability, and referral arrangements needed to ensure continuity of health care. Primary Care includes longitudinal outpatient medical care for the purposes of prevention and detection of disease and subsequent management of medical conditions in eligible veterans. The care will be provided by primary care providers who may be general practitioners or general internists or their designees. The care provided should be continuous, accessible, and coordinated. This care will include: (1) Scheduled initial or follow-up visits to primary care providers at the CBOC site. Appointments must be held within seven (7) days of request by established patients and within 30 days of enrollment for new patients. Within the first two (2) visits, a complete history and physical examination will be performed with documentation of patient problems via the on-line Problem List (Graphical User Interface Computer Patient Records System problem list tab) option in the Veterans Health Information Systems and Technology Architecture (VISTA) computer system which will be updated as needed on each subsequent visit. A complete history and physical examination must be performed on each member every three (3) years thereafter for the duration of the contract. (2) Office visits to other health care providers including nurses or physician extenders for the purposes of monitoring disease, patient education or prevention measures. (3) Diagnostic tests ordered by primary care provider or his/her designee as indicated in paragraph 1.b. below. (4) Phone contacts with patients and primary care providers or their designee. The solicitation will be issued on/about June 16, 2009, with a proposal response expected to be July 16, 2009. Offers will be accepted from only responsible and responsive contractors currently engaged in the type of work being required by this solicitation. Technical Evaluation Criteria for this solicitation are as follows: Technical Excellence, Past Performance, Price/Cost.
