Description
QASP PBSOW (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation number 09-235-SOL-00011 is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-33. (iv) This acquisition is a Total Small Business Set-Aside. The NAICS code is 541990 and the size standard is $7,000,000. (v) There is only one contract line item which is to provide Diabetes Case Management in accordance to the scope of work. (vi) The contractor shall provide all services required in the Performance Based Statement of Work (PBSOW). (Attached) The contractor’s performance of the contract will be evaluated by the criteria set forth in the Quality Assurance Surveillance Plan (QASP). (Attached) (vii) The period of performance is July 23, 2009 through July 22, 2010. (viii) The provision 52.212-1, Instructions to Offerors—Commercial, applies to this acquisition. (ix) The provision 52.212-2, Evaluation—Commercial Items, applies to this acquisition. The following factors shall be used to evaluate offers: price, technical capability, and past performance. Technical and past performance, when combined, are significantly less important than price. (x) Offerors shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications—Commercial Items, with its offer. (xi) The clause 52.212-4, Contract Terms and Conditions—Commercial Items, applies to this acquisition. (xii) The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders—Commercial Items, applies to this acquisition. (xiii) There are no additional contract requirements. (xiv) The Defense Priorities and Allocations System (DPAS) do not apply to this acquisition. (xv) Quotes are due by July 2, 2009, 5:00 PM P.S.T.
