Evaluation Criteria Past Performance Questionnaire SOW THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6-STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL ITEMS-AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTATIONS ARE BEING REQUESTED, AND A WRITTEN SOLICITATION DOCUMENT WILL NOT BE ISSUED. THIS SOLICITATION IS BEING ISSUED USING SIMPLIFIED ACQUISITION PROCEDURES 13.5. This solicitation is a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-32. The associated North American Industrial Classification System (NAICS) code for this procurement is 541611 is set aside for small businesses only. 1. TITLE Feasibility Study to help the National Institutes of Health Identify and maintain relevant Sampling Frames. 2. BACKGROUND The National Institutes of Health (NIH), a part of the U.S. Department of Health and Human Services, is the primary Federal agency for conducting and supporting medical research. Helping to lead the way toward important medical discoveries that improve people's health and save lives, NIH scientists investigate ways to prevent disease as well as the causes, treatments, and even cures for common and rare diseases. Composed of 27 Institutes and Centers, the NIH provides leadership and financial support to researchers in every state and throughout the world. The NIH annually invests over $28 billion in medical research. More than 83% of the NIH's funding is awarded through almost 50,000 competitive grants to more than 325,000 researchers at over 3,000 universities, medical schools, and other research institutions in every state and around the world. About 10% of the NIH's budget supports projects conducted by nearly 6,000 scientists in its own laboratories, most of which are on the NIH campus in Bethesda, Maryland. NIH leadership frequently requires feedback and insights from various constituencies about the effects and operation of various NIH policies and programs. Scientific societies, professional organizations, and advocacy groups serve as useful resources when seeking the views of constituents. Additionally, the NIH has asked for input from advisory groups, has issued Requests for Information (RFI), and in cases involving the development of regulations the NIH has issued Notices of Proposed Rulemaking (NPRM). These approaches, however, may tend to filter out minority opinions and observations, and miss obscure facts. Additionally, the input may not target or adequately represent the constituencies most affected by contemplated policy. Finally, the comments are always unstructured and frequently difficult to analyze. 3. Project Description The NIH seeks guidance related to the feasibility of developing general procedures for conducting surveys and for identifying and assembling suitable sampling frames to collect structured and representative information from constituencies of interest. This feasibility study will help the NIH move quickly to identify and use existing frames or to develop and refresh sampling frames as required, and it will help the NIH assemble the resources necessary to develop and field suitable surveys that employ methodologically sound survey and sampling designs, power analyses, questionnaire construction, follow-up procedures that yield high response rates, data analytical strategies, streamlined OMB clearance procedures, and guidance on factors that will necessitate IRB review. 4. Technical Evaluation Criteria See Attachment III, Technical Evaluation Criteria 7. PERIOD OF PERFORMANCE. The period of performance is not to exceed twelve (12) months from the date of award of the contract. Period of Performance: 6 - 12 months “Base Year” QUESTIONS DEADLINE: all questions must be received by email (stevenst@od.nih.gov) before 2:00 pm (1400) EST on 22 June 2009. Delivery terms shall be FOB Destination. FOB Destination means the contractor shall pack and mark the shipment in conformance with carrier requirements, deliver the shipment in good order and condition to the point of delivery specified in the purchase order, be responsible for loss of and/or damage to the goods occurring before receipt and acceptance of the shipment by the consignee at the delivery point specified in the purchase order; and pay all charges to the specified point of delivery. Award will be made to the party whose quote offers the best value to the Government, technical, price, and other factors considered. The Government may award this contract to other than the lowest price technically acceptable quote. The Government will evaluate information based on the following evaluation criteria: 1) Technical Capability factor described to "Meet or Exceed the Requirement," 2) Past Performance, and 3) Price. Technical Capability and Past Performance is more important than price. Evaluation of Technical Capability shall be based on the information provided in the quotation against the evaluation criteria. Quoters shall include their understanding of the requirement and any past performance that can support your quote or other literature which CLEARLY DOCUMENTS that the offered service meets or exceeds the specifications stated herein. This announcement constitutes the only solicitation and a written solicitation will not be issued for the development of information management processes as defined herein. The Government intends to award a Fixed Price under Simplified Acquisition Procedures. Additionally, all quotes shall include a technical volume not exceeding 15 pages (not including resumes & past performance), and a price volume; FOB point; a poc (name and telephone number); a statement from the offeror verifying that they are CCR Registered under NAICS code 541611; delivery date; business size; and payment terms. The Offeror must include a completed copy of the following provisions: 1) FAR Clause 52.212-1 Instructions to Offerors Commercial; 2) FAR Clause 52.212-2, Evaluation Commercial Items. As stated in FAR Clause 52.212-2 (a), the Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered; 3) FAR Clause 52.212-3, Offeror Representations and Certifications Commercial Items; 4) FAR Clause 52.212-4, Contract Terms and Conditions Required To Implement Statues or Executive Orders Commercial Items, Contract Terms and Conditions Commercial Items; and 5) FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items Deviation for Simplified Acquisitions. The Offeror must include in their quotation the unit price, the list price, shipping and handling costs, the delivery period after contract award, the prompt payment discount terms, the F.O.B. Point (Destination or Origin), the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. Note: In order to receive an award, offerors must have a valid registration in the Central Contractor Registration (CCR) www.ccr.gov. Interested parties capable of furnishing the Government with the requirement specified in this synopsis must submit their quotation to the below address.
