- Appendix A - ARRA Building Plan Check Scope of Work The Stanford Linear Accelerator Center (SLAC) Building Inspection Office (BIO) is soliciting Proposals for a Subcontractor for ARRA related Plan Check and Building Inspection Services. The awarded subcontract will be a subcontract under Prime Contract No. DE-AC02-76SF00515 between the University and the United States Government (hereinafter called "Government"), represented by the Department of Energy (hereinafter called "DOE") for the management and operation of the SLAC National Accelerator Laboratory (hereinafter called “SLAC”) and the performance of certain research and development work. The successful Offeror shall possess in-depth experience, have a local (Bay Area) presence, and provide professional services for the review of proposed commercial and industrial (structural and non-structural) buildings, including electrical, plumbing, mechanical, public works and earthworks plans; and provide building inspections in conformance with all applicable regulatory requirements. Such regulatory requirements shall include, but not be limited to current Building, Disabled Access, Plumbing, Mechanical, and Electrical Codes, as well as all Federal-level and other laws to which SLAC is subject, including 10 CFR 851. Proposal Contents: Price Schedule for services •Statement of Qualifications describing why your company is qualified to perform the Scope of Work, including an organizational chart showing the locations of current full-time offices and staff members. It is preferred that the successful Subcontractor have experience in San Mateo or Santa Clara County municipal building departments, and/or Federal Government facilities similar to SLAC. •A Service Approach Statement indicating an overall understanding of the Scope of Work. •A Methodology and Ability to Perform Statement explaining how the Subcontractor is able to perform plan reviews and field inspections. •Three or Four sample building plan check reviews for large construction projects •A sample invoice/monthly bill for a sample plan check (black out for proprietary and confidential information is permitted and preferred). •Resumes of each proposed staff member, and the names and roles of the Project Manager and personnel to be assigned to the project, including years of employment with the company. ICC Certification of assigned personnel performing the plan reviews or inspection services is required. •Provide a list with the names, experience and qualifications of the assigned personnel in terms of the Scope of Work (i.e., Building and Fire Code Plans Examiner, Combination Inspector, and registered professional Structural Engineer, etc.). •List specific tasks to be performed by each proposed staff member. •Demonstrate the ability to provide web-based, real-time plan review comments and current ability to perform electronic plan review. •Demonstrate the ability to meet plan check turnaround time for standard and accelerated plan checks. The time period begins when plans are submitted to SLAC, and ends when the applicant receives the comments. This time frame shall include all travel time. The Subcontractor shall receive the plans no later than the day after SLAC receives them. •Provide a list of project references for recent, similar work. This list should contain at least three projects that are similar in scope and include the project name, date(s), address, client contact (i.e. Project Manager, City Manager, and Building Official), telephone number of the client contact, and the Subcontractor’s Project Manager. This project list must also include the names of the staff members who worked on this project and whose resumes are included in the Request for Proposal. •Demonstrate the willingness and ability to provide the required amount of insurance coverage as required by SLAC, within ten (10) calendar days of notification of selection for award of this Request for Proposal. •Provide a schedule of cost and fees, including the hourly billing rate for each of the proposed staff members. Include all direct and in-direct costs associated with services being provided, including project management, reproduction costs, delivery costs, transportation, etc., and describe how these costs will be billed (i.e., are they included in the hourly labor rate or billed separately). •Demonstrate the ability to provide on-call inspection and plan check services on short notice (less than 24 hours), using subject matter experts described under this Scope of Work.
