Description
This is a combined synopsis/solicitation for commercial item prepared in accordance with the formation in Subpart 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation is issued as a request for quotation (RFQ) under solicitation number DTMA1Q09049. This solicitation document and incorporated provision/clauses are those in effect through Federal Acquisition Circular (FAC) 2005-31 (2009). This requirement is full and open and is not restricted to business size and the NAICS code is 483111. The Department of Transportation (DOT) Maritime Administration (MARAD) United States Merchant Marine Academy (USMMA) 300 Steamboat Road, Kings Point, NY 11024 intends to procure service to provide supplemental crew for T/V Kings Pointer summer training/shipyard voyage and provide engineering assistance during a 30 day repair period. Engineering assistance during a 30 day as specified in the below associated Statement of Work (SOW) on a firm-fixed price basis: STATEMENT OF WORK (SOW):1. Provide two (2) mates with current unlimited tonnage licenses and current STCW certificates with training vessel/environment experience preferred for voyage from Kings Point, New York to Jacksonville, Florida. As per the vessel Certificate of Inspection (COI) it does not matter if the mates are Chief Mates, 2nd Mates or 3rd Mates. The two (2) unlimited mates are required to report on board the vessel by 23 June 2009 at 08:00 a.m. and discharge from vessel in Jacksonville, Florida on or about 30 June 2009. 2. Provide three (3) engineers with current unlimited horsepower licenses and current STCW certificates with training vessel/environment experience preferred for voyage from Kings Point, New York to Jacksonville, Florida. As per the vessel Certificate of Inspection (COI) it does not matter if the engineers are 1st Asst Eng, 2nd Asst Eng or 3rd Asst Eng. The three (3) unlimited horsepower engineers are required to report on board by 23 June 2009 at 08:00 a.m. and discharge from vessel on or about 30 June 2009. Option 2a. Provide one (1) engineer from the three engineers to remain with the ship during the shipyard period and return with the ship to Kings Point, New York on or around 01 August 2009. Per diem, lodging and subsistence will be in accordance with Federal Travel Regulations, Travel Cost, FAR subpart 31.205-46 3. All travel will be in accordance with Federal Travel Regulations, Travel Cost FAR subpart 31.205-46. NOTE: Mates and engineers will be required to have a TWIC card and report to the KINGS POINTER vessel on or around 23 June 2009 by 08:00 a.m. Please contact Captain Tom Dorr at (516) 773-5680 for access. The following FAR provisions and clauses apply to this solicitation and are incorporated by reference: 52.212-1, Instructions to Offerors - Commercial Items (JUN 2008), 52.212-2, Evaluation-Commercial Items (JAN 1999). The Government will award a contract resulting from this solicitation to the responsibility offeror whose offer to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate: (1) price, (2), past performance, and (3) technically acceptable. Technical and past performance when combined are equal when compared to price. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The government may determine that an offer is unacceptable if the option prices are significantly unbalanced.; 52.212-3 Offeror representations and Certifications-Commercial Items (FEB 2009). An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (m) of this provision. After reviewing the ORCA database information, the offeror verifies by submission of this offer that the representation and certifications currently posted electronically at FAR 52.212-3, Offeror Representations and Certifications Commercial Items have been entered or updated in the last 12 months, are current, accurate, complete, and applicable to this solicitation including the NAICS code referenced for this solicitation, as of the date of this offer and are incorporated in this offer by reference; 52.212-4 Contract Terms and Conditions-Commercial Items (MAR 2009) and 52.212-5 Contract Terms and Conditions Required To Implement Statutes or Executive Orders (APR 2009) (applies to this acquisition; Within 52.212-5 Contract Terms and Conditions Required To Implement Statutes or Executive Orders, the following clauses apply and are incorporated by reference: 52.203-06 Restrictions On Subcontractor Sales to the Government (SEPT 2006), 52.222-3, Convict Labor (JUN 2003), 52.222-21 Prohibition of Segregated Facilities (FEB 1999), 52.222-26, Equal Opportunity (MAR 2007), 52.222-36 Affirmative Action for Workers with Disabilities (JUN 1998), 52.225-13 Restrictions on Certain Foreign Purchases (JUN 2008), 52.232-33 Payment by Electronic Funds Transfer-Central Contractor Registration (OCT 2003), 52.222-41 Service Contract Act of 1965 (NOV 2007). Parties responding to this solicitation may submit their quotation along with their technical approach explaining how the SOW will be accomplished in accordance with their standard commercial practice (e.g. company letterhead, formal quote form etc.) but MUST include the following information: 1) Complete company name, mailing and remittance addresses, 2) Prompt payment terms, 3) Delivery schedule, 4) Taxpayer ID number, 5) Duns# and Cage Code, 6) ALL COMPLETED CERTIFICATIONS AS REQUIRED HEREIN - SPECIFICALLY THOSE FOUND IN 52.212-3. All deliverables shall be FOB destination. Responses and questions to this Request for Quotation (RFQ) should be directed to the Department of Transportation via email to janice.marshall@dot.gov or fax to 202-366-3237 NO LATER THAN 19 JUNE 2009 BY 11:00 A.M.. Reference: DTMA1Q09049 on your RFQ. Award will be fixed price. Award will be made to the lowest priced responsible offeror submitting a quote which conforms to this RFQ.
