Description
COMBINED SYNOPSIS/SOLICITATION NOTICEACIST Auto Injector System and Accessories/Equipment This is a combined synopsis/solicitation for commercial requirements prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the ONLY solicitation; a written solicitation will NOT be issued. This simplified acquisition procurement is also being accomplished in accordance with FAR Part 6.302- 1. The Department of Veterans Affairs (VA), Southern AZ VA Health Care System (SAVAHCS), Office of Acquisitions and Logistics, Equipment Section, 3601 S. 6th Avenue Tucson, Arizona intends to negotiate sole-source for an Automatic Injector System and Accessories with Acist Medical Systems of 7905 Fuller Road Eden Prairie MN 55344. The Automatic Injector System, a CVi System, is crucial to the operation of an advanced interventional cardiology catheterization laboratory within the SAVAHCS. The system will allow interventional cardiologists to perform interventional procedures in a more streamlined and efficient manner. The system allows processes to be simplified and ensures the adequate amount of contrast is injected, as appropriate. This system may also eliminate the requirement of an extra FTEE during after-hours. The system includes built-in safety features, which benefit patient care. Majority of the current SAVAHCS cardiologists are already trained on the usage of this system. Along with the Auto Injector System, SAVAHCS requires the Hemodynamic Monitoring Cables, the Interface Cables, the Consumables that are compatible to the equipment, installation services, training, and extended warranty service of such systems. The applicable NAICS for this requirement is 339112. The Government will award a Firm-fixed price contract resulting from this solicitation. The Contractor shall provide a complete copy of the Offeror Representations and Certifications-Commercial Items (FAR 52.212-3 June 2009) with its offer or complete the annual representations and certifications electronically at http://orca.bpn.gov. Many of the provisions and clauses cited in this solicitation are incorporated by reference, FAR 52.252-2 (FEB 1998), to the appropriate section of the FAR and VAAR. A review of these provision and clauses will be necessary for you to understand all aspects of the solicitation. The full text of any FAR provision and clause can be found at http://www.arnet.gov/far and http://www.va.gov/oa&mm/vaar/. The following applicable FAR clauses apply: Instruction to Offerors-Commercial Items (FAR 52.212-1 JUNE 2008); Offeror Representations and Certifications-Commercial Items (FAR 52.212-3 JUNE 2009); Contract Terms and Conditions-Commercial Items (FAR 52.212-4 MAR 2009); Addendum to FAR 52.212-4; Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (FAR 52.212-5 JUNE 2009), including the following clauses: Restrictions on Subcontractor Sales to the Government (ALT I FAR 52.203-6 SEPT 2006), Utilization of Small Business Concerns (52.219-8 MAY 2004), Equal Opportunity (FAR 52.222-26 MAR 2007), Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era (FAR 52.222-35 SEPT 2006), Affirmative Action for Workers with Disabilities (FAR 52.222-36 JUN 1998), Employment Reports on Disabled Veterans and Veterans of the Vietnam Era (FAR 52.222-37 SEPT 2006), Correspondence (VAAR 852.236-76 APR 1984), Contractor's Responsibilities (VAAR 852.237-70 JAN 2008), Commercial Advertising (VAAR 852.203-70 JAN 2008), Representatives of Contracting Officers (VAAR 852.270-1 JAN 2008).
